医学影像融合配准技术在前列腺癌治疗中的应用

Medical image registration and fusion technology applied in prostate cancer radiotherapy

摘要 目的探讨肿瘤放射治疗中,采用CT/MRI图像融合配准技术在前列腺癌靶区精准勾画中的应用。方法选取2018年1月-2019年1月来濮阳市安阳地区医院治疗的前列腺癌患者86例为研究对象,将CT扫描图像和MRI扫描图像,传送到图像融合软件(Oncentra),并对CT/MRI融合图像上确定的靶区体积和危及器官受量与单纯CT定位图像直接勾画的靶区体积和危及器官受量进行对比。结果对比结果显示,经定位CT图像直接勾画靶区确定的肿瘤平均体积为71.62 mm^3,而经过CT/MRI图像融合确定的肿瘤平均体积为52.23 mm^3。可见采用CT/MRI图像融合配准技术更能精准确定前列腺癌放疗靶区的范围。并且对两种方法所致危及器官(膀胱及直肠)照射剂量进行对比,结果显示医学图像融合技术勾画的靶区所致危及器官的最大及最小照射剂量均小于单纯CT图像所勾画的靶区,且P≤0.05差异具有统计学意义。结论在肿瘤放射治疗中采用医学图像融合配准技术能更精准勾画前列腺癌的靶区,从而降低危机器官受量,有利于精准治疗,在肿瘤放射治疗中有较高应用价值。 Objective To explore the application of CT/MRI image fusion registration technology in the accurate delineation of prostate cancer target areas in tumor radiotherapy.Methods The 86 cases of prostate cancer patients treated in our hospital from January 2018 to January 2019 were selected as the research subjects,and the CT and MRI image data of 86 patients were processed using Oncentra Master Plan.And compare the target volume and endangered organ volume determined on the CT/MRI fusion image with the target volume and endangered organ volume directly delineated by the CT localization image.Results The contrast results showed that the volume of the tumors determined by CT images was 71.62 mm^3, while that determined by CT/MRI image fusion registration technology was 52.23 mm^3. It can be seen that the CT/MRI image fusion registration technology can more accurately determine the range of prostate cancer radiotherapy target area.And compare the doses of organs(bladder and rectum) caused by the two methods, the results showed that the maximum and minimum radiation doses of medical image fusion technology were less than those of CT images, and the difference was statistically significant(P≤0.05).Conclusion The use of medical image fusion registration technology in tumor radiotherapy can more accurately outline the target area of prostate cancer, thereby reducing the dose of crisis organs, which is conducive to precise treatment, and has high application value in radiotherapy.
