不同类型膝关节假体对初次单侧膝关节置换患者术后患肢功能恢复的影响

Influences of different knee joint prosthesis types on limb function recovery after initial and unilateral knee arthroplasty

目的观察初次单侧膝关节置换手术行旋转平台与后稳固定平台的临床疗效,以及对术后膝关节肢体肿胀情况以及患肢膝关节肢体功能恢复的影响。方法纳入2016年12月-2018年12月收治膝关节骨关节炎实施全膝关节置换术者160例作为研究对象,根据置换膝关节假体类型不同分为旋转组和固定组,每组80例。旋转组应用旋转平台假体实施初次单侧全膝关节置换手术,固定组应用后稳固定平台实施初次单侧全膝关节置换手术。观察两组患者术后血清炎性水平以患肢膝关节肢体功能恢复,并对比两组临床疗效。结果旋转组和固定组治疗后血清中C反应蛋白及IL-6水平均低于治疗前,差异有统计学意义(P<0.05)。旋转组和固定组治疗后膝关节功能评分均高于治疗前,差异有统计学意义(P<0.05)。旋转组出院后3个月和出院后6个月膝关节功能评分均优于固定组,差异有统计学意义(P<0.05)。结论旋转平台膝关节假体行初次单侧膝关节置管手术可提高患者术后患肢功能,且降低血清炎性反应。

Objective To compare the clinical efficacy of cruciate retaining and posterior stablized prosthesis during initial and unilateral knee arthroplasty,as well as their influences on limb swelling and limb function recovery.Methods A total of160 knee osteoarthritis patients who underwent total knee arthroplasty from December2016 to December2018 were selected and assigned to cruciate retaining group and posterior stablized group according to knee joint prosthesis types,with80 cases in each group. Patients adopted initial and unilateral knee arthroplasty with cruciate retaining and posterior stablized prosthesis,respectively. Their postoperative serum inflammatory response,limb function recovery,and clinical efficacy were assessed. Results After respective treatments,the serum C-reactive protein and IL-6 levels of the two groups were lower than before(P<0.05). The knee joint function scores of the two groups after respective treatments were higher than before(P<0.05). The knee joint function scores of the cruciate retaining group3 and6 months after discharge were higher than those of the posterior stablized group(P<0.05).Conclusion During initial and unilateral knee arthroplasty,the cruciate retaining prosthesis can improve limb function and reduce serum inflammatory response.
