听神经瘤术后并发症的影响因素分析 被引量:6

摘要 目的探讨听神经瘤患者术后并发症的发生率和影响因素。方法回顾性分析南京医科大学第一附属医院神经外科2018年1月—2019年9月收治的96例行显微外科治疗的听神经瘤患者的临床资料。结果本组患者以头晕(66.7%)、听力下降(58.3%)和面瘫(32.3%)为最常见的听神经瘤术后并发症;脑脊液漏(10.4%)是术后严重的并发症。肿瘤大小、手术时长、复发肿瘤和乳突气房的开放是影响脑脊液漏发生率的独立危险因素(均P<0.05)。颅内感染(8.3%)的发生与手术时长和脑脊液漏有关(均P<0.05)。结论显微外科手术对听神经瘤有较好的治疗效果,在围手术期针对上述危险因素实施防治措施,对患者预后有着重要意义。 Objective To investigate the incidence and related factors of postoperative complications in acoustic neuroma patients.Methods The clinical data of 96 patients with acoustic neuroma underwent microsurgery in the First Affiliated Hospital of Nanjing Medical University from January 2018 to September 2019 were analyzed retrospectively.Results In this data,dizziness(66.7%),hearing impairment(58.3%),and facial paralysis(32.3%)were the most common postoperative complications.Cerebrospinal fluid leakage(10.4%)was a serious complication after acoustic neuroma surgery.Tumor size,operation time,tumor recurrence,and eradicate the mastoid process were independent risk factors of the cerebrospinal fluid leakage(all P<0.05).The incidence of intracranial infections(8.3%)was related to operative time and cerebrospinal fluid leakage(all P<0.05).Conclusion Microsurgery is effective in the treatment of acoustic schwannoma.The prevention and treatment of these risk factors in perioperative period is efficient to the prognosis of patients.
