胰岛素联合低分子肝素治疗高脂血症性急性胰腺炎的临床效果及安全性分析

Clinical effectiveness and safety analysis of insulin combined with low molecular weight heparin on the treatment of patients with hyperlipidemic acute pancreatitis

[目的]分析胰岛素联合低分子肝素治疗高脂血症性急性胰腺炎(HLAP)的临床效果及安全性。[方法]选取2016年6月~2019年4月我院收治的HLAP患者104例,按随机数字法分为A、B、C、D组,A组给予常规治疗;B组常规治疗基础上再给予胰岛素治疗;C组常规治疗基础上再给予低分子肝素治疗;D组常规治疗基础上再给予低分子肝素及胰岛素联合治疗。治疗后观察及评价各组治疗效果。[结果]4组患者治疗前C反应蛋白、三酰甘油及血清淀粉酶水平差异无统计学意义;治疗9 d后D组患者C反应蛋白、三酰甘油及血清淀粉酶水平显著性低于其他3组(P<0.05);治疗14 d后,D组治疗总有效率为92.3%,显著性高于其他3组(P<0.05)。[结论]胰岛素联合低分子肝素治疗HLAP的临床效果满意,安全性好,建议在临床治疗中推广应用。

[Objective] To analyse the clinical effect and safety of insulin combined with low molecular weight heparin(LMWH) on the treatment of patients with hyperlipidemic acute pancreatitis.[Methods]One hundred and four patients with hyperlipidemic acute pancreatitis in our hospital from June 2016 to April 2019 were collected and divided into group A,group B,group C and group D,which were treated with regular treatment.The patients in B group were treated with insulin in the basis of regular treatment.The patients in C group were treated with LMWH in the basis of regular treatment.The patients in D group were treated with insulin and LMWH in the basis of regular treatment.Clinical effectiveness was observed after treatment.[Results]The CRP,TG and AMY levels in the four groups had no significantly difference before the treatments(P>0.05).The CRP,TG and AMY levels in the D group were significantly lower than these in other three groups after treatments(P<0.05).The total effective rate in D group was 92.3%,which was significantly higher than these in other three groups after treatments(P<0.05).[Conclusion]The clinical effectiveness of insulin combined with LMWH on the treatment of patients with hyperlipidemic acute pancreatitis is effective.
