比较血清与痰液样本中降钙素原检测在儿童细菌性呼吸道感染中的差异 被引量:1

摘要 目的比较血清与痰液样本降钙素原(PCT)检测在儿童细菌性呼吸道感染中的差异。方法选取2018年1月-10月收诊的100例呼吸道感染患者作为观察对象,随机分为细菌感染组与非细菌感染组,各50例,选择体检健康儿童50例为对照组。分别进行血清、痰液样本中PCT检测,分析血清及痰液的特异度及灵敏度,观察细菌感染组治疗后PCT动态变化与疾病缓解率的关系,并分析PCT浓度与患儿预后的关系。结果细菌感染组患儿痰液PCT水平显著高于血清PCT水平,差异有统计学意义(P<0.05);细菌感染组患儿PCT值与疾病缓解率呈负相关(P<0.05);血清、痰液PCT浓度越高,患儿的发热、咳嗽、抗生素使用时间及住院时间明显较长(P<0.05);痰液PCT诊断的灵敏度、特异度显著高于血清PCT(P<0.05)。结论相比血清PCT检测,痰液PCT检测的阳性率、灵敏度、特异性更高,值得临床进一步的探索研究。 Objective To compare procalcitonin(PCT)detection between serum and sputum samples in children with bacterial respiratory infection.Methods A total of 100 patients with respiratory infection admitted between January and October in 2018 were selected as the observation objects,and randomly divided into bacterial infection group and non-bacterial infection group,with 50 cases in each group.50 healthy children were selected as the control group.PCT detection was performed in serum and sputum samples.The specificity and sensitivity of serum and sputum were analyzed to observe the relationship between PCT changes and disease remission rate in bacterial infection group and to analyze the relationship between PCT concentration and prognosis.Results The level of sputum PCT in bacterial infection group is significantly higher than that in serum PCT level and positive rate,with the differences statistically significant(P<0.05).The PCT value of children with bacterial infection was negatively correlated with disease remission rate(P<0.05).The time of fever,cough,the application time of antibiotics and hospitalization time were significantly longer in children with high PCT concentration(P<0.05).The sensitivity and speci-ficity of sputum PCT for diagnosing lower respiratory tract infection were significantly higher than those of serum PCT(P<0.05).Conclusion Compared with serum PCT detection,sputum PCT detection has higher positive rate,sensitivity and spe-cificity,which is worthy of further clinical research.
