剥脱性与非剥脱性点阵激光治疗痤疮瘢痕的效果比较

Comparison effect of exfoliative and non-exfoliative fractional laser on acne scars

目的探讨剥脱性与非剥脱性点阵Er∶YAG激光在治疗痤疮瘢痕的效果差异。方法选择痤疮瘢痕患者220例,其中男性135例,女性85例;年龄18~35岁,平均年龄24.6岁。随机分为剥脱组和非剥脱组各110例。剥脱组男性60例,女性50例;年龄18~33岁,平均年龄24.5岁。非剥脱组男性75例,女性35例;年龄20~35岁,平均年龄25.3岁。剥脱组采用2940 nm Er∶YAG点阵激光,脉宽0.4 ms,光斑0.4 mm,单脉冲能量50 mJ;非剥脱组采用波长1540 nm晶钻点阵激光,脉宽10 ms,光斑10 mm,单脉冲能量50 mJ;1个月治疗1次,间隔1个月再次治疗1次。比较两组临床疗效、总疗程、治疗次数和并发症。结果两组患者的临床疗效比较,差异无统计学意义(90.9%vs 89.1%;χ^2=0.202,P=0.653)。两组总疗程、治疗次数比较,差异无统计学意义[总疗程:(5.2±1.3)个月vs(5.0±1.4)个月;t=0.632,P=0.217]。两组并发症发生率比较,差异无统计学意义(9.1%vs 11.8%;χ^2=0.437,P=0.509)。结论剥脱性与非剥脱性点阵Er∶YAG激光治疗痤疮瘢痕的总体效果相当。

Objective To investigate difference of effect between exfoliative and non-exfoliative lattice Er:YAG laser in treatment of acne scars.Methods A total of 220 patients with acne scar were enrolled,which included 135 males and 85 females,aged 18-35 years old with mean age of 24.6 years old.All of them were randomly divided into exfoliation group(n=110,which included 60 males and 60 females,aged 18-33 years old with mean age of 24.5 years old,performed 2940 nm Er∶YAG fractional laser,pulse width of 0.4 ms,spot size of 0.4 mm,single pulse energy of 50 mJ)and non-exfoliation group(n=110,which included 75 males and 35 females;aged 20-35 years old with mean age of 25.3 years old,performed crystal fractional laser wavelength of 1540 nm,pulse width of 10 ms,spot size of 10 mm,and single pulse energy of 50 mJ).The treatment was performed once a month,then a month later the second treatment was performed.The clinical efficacy,total treatment time,frequency and complications were compared between 2 groups.Results There was no significant difference in clinical efficacy between 2 groups(90.9%vs 89.1%;χ^2=0.202,P=0.653).There was no significant difference in total treatment time between 2 groups[(5.2±1.3)months vs(5.0±1.4)months;t=0.632,P=0.217].There was no significant difference in complication incidence between 2 groups[9.1%vs 11.8%;χ^2=0.437,P=0.509].Conclusion It is demonstrated that the overall effect of exfoliative and non-exfoliative Er∶YAG fractional laser in treatment of acne scars is comparable.
