强直性脊柱炎全髋关节置换术临床疗效分析

Clinical Analysis of Total Hip Replacement for Ankylosing Spondylitis

目的分析强直性脊柱炎行全髋关节置换术的临床疗效。方法从2015年1月—2019年2月期间我院收治的强直性脊柱炎患者中选择60例作为分析对象,按治疗方式的不同将其分为两组,予以骶髂关节针刀松解术治疗的30例患者归入对比组;予以全髋关节置换术治疗的30例患者归入研究组。比较两组治疗前后的髋关节Harris评分系统(HHS)评分、并发症发生率及治疗效果。结果治疗前,两组患者的HHS评分无统计学差异(P>0.05),经过治疗后,研究组的HHS评分高于对比组(P<0.05);研究组的并发症发生率为3.33%,明显低于对比组的23.33%(P<0.05);研究组的治疗总有效率为93.33%,明显高于对比组的73.33%(P<0.05)。结论实行全髋关节置换术治疗强直性脊柱炎患者,能有效改善其髋关节活动功能,且下肢深静脉血栓、异位骨化等并发症出现概率更低,治疗效果更好。

Objective To analyze the clinical effect of total hip replacement for ankylosing spondylitis.Methods From January 2015 to February 2019,60 patients with ankylosing spondylitis in our hospital were selected as the objects of analysis.According to the different treatment methods,they were divided into two groups.30 patients who were treated with sacroiliac joint needle knife release were classified into the control group;30 patients who were treated with total hip replacement were classified into the study group.The Harris scoring system(HHS)score,complication rate and therapeutic effect of the two groups were compared before and after treatment.Results Before treatment,there was no significant difference in HHS score between the two groups(P>0.05).After treatment,the HHS score of the study group was higher than that of the control group(P<0.05);the complication rate of the study group was 3.33%,significantly lower than 23.33%(P<0.05);the total effective rate of the study group was 93.33%,significantly higher than 73.33%(P<0.05).Conclusion Total hip arthroplasty can effectively improve the hip function of patients with ankylosing spondylitis,and the incidence of complications such as deep vein thrombosis and heterotopic ossification of the lower extremity is lower,and the treatment effect is better.
