独一味联合快速康复训练对老年全膝关节置换术患者运动功能、脂质蛋白及炎症指标的影响

Effect of Duyiwei capsule combined with rapid rehabilitation training on motor function,lipid protein and inflammatory indicators in elderly patients with total knee arthroplasty

目的探究独一味联合快速康复训练对老年全膝关节置换术(TKA)患者运动功能、脂质蛋白及炎症指标的影响。方法前瞻性选择行TKA患者98例,依据术后治疗方法不同分为研究组(n=51)与对照组(n=47),对照组采用快速康复训练,研究组在对照组基础上服用独一味胶囊,两组均连续治疗14 d。比较两组术前和术后14 d时视觉模拟评分(VAS)、特种外科医院膝关节功能评分(HSS)、健康调查简表(SF-36)评分及血清低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、三酰甘油(TG)、总胆固醇(TC)、白细胞介素(IL)-1β、肿瘤坏死因子(TNF)-α、IL-1水平,比较两组术后并发症发生率。结果术前两组VAS、HSS评分、SF-36评分及血清LDL-C、HDL-C、TG、TC、IL-1β、TNF-α、IL-6水平差异无统计学意义(P>0.05);术后14 d两组VAS及血清TG、IL-1β、TNF-α、IL-6水平显著低于术前(P<0.05),HSS、SF-36评分及血清HDL-C水平显著高于术前(P<0.05),血清LDL-C、TC水平较术前差异无统计学意义(P>0.05);术后14 d研究组VAS及血清TG、IL-1β、TNF-α、IL-6水平显著低于对照组(P<0.05),HSS、SF-36评分及血清HDL-C水平显著高于对照组(P<0.05),血清LDL-C、TC水平较对照组差异无统计学意义(P>0.05);研究组术后并发症发生率显著低于对照组(P<0.05)。结论独一味联合快速康复训练可促进老年TKA患者运动功能恢复,改善脂质蛋白及炎性因子水平,且术后并发症少。

Objective To investigate the effect of Duyiwei capsule combined with rapid rehabilitation training on motor function,lipid protein and inflammatory indicators in elderly patients with total knee arthroplasty(TKA).Methods 98 elderly patients undergoing TKA were prospectively selected and divided into two groups according to the different treatment methods.The control group(n=47)used rapid rehabilitation training,and the study group(n=51)received the Duyiwei capsule combined with rapid rehabilitation training.After 14 days of continuous treatment,various indexes were compared between the two groups,including visual analogue scale(VAS)score,Hospital for Special Surgery(HSS)knee score,the MOS 36-Item Short Form Health Survey(SF-36)score,serum low-density lipid cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),triglyceride(TG),total cholesterol(TC),interleukin(IL)-1β,tumor necrosis factor(TNF)-α,IL-1 levels,and the incidence of postoperative complications.Results VAS score,HSS score,SF-36 score and serum LDL-C,HDL-C,TG,TC,IL-1β,TNF-αand IL-6 levels showed no difference between the two groups(P>0.05).At the postoperative 14 d,the VAS scores and serum TG,IL-1β,TNF-αand IL-6 levels were decreased in both groups(P<0.05),and were lower in the study group than those in the control group(P<0.05),meanwhile,the HSS score,SF-36 score and serum HDL-C level were significantly increased(P<0.05),and were higher in the study group than those in the control group(P<0.05).Serum LDL-C and TC levels of two groups showed no difference before and after operation(P>0.05).The incidence of postoperative complications was significantly lower in the study group than that in the control group(P<0.05).Conclusions Duyiwei capsule combined with rapid rehabilitation training could effectively promote the recovery of motor function,ameliorate the levels of lipid protein and inflammatory indicators for elderly patients undergoing TKA,with fewer complications.
