摘要
目的研究关节软骨中整合素(ITG)αvβ3表达水平与老年膝骨关节炎退变程度的相关性。方法回顾性分析20例老年膝骨关节炎患者行全膝关节置换术得到的不同退变程度的71个关节软骨标本,根据Bobinac分期和苏木素-伊红(HE)染色后Mankin评分将标本分为正常组(n=5)、轻度退变组(n=23)、中度退变组(n=24)、重度退变组(n=19),利用qRT-PCR、Western印迹检测不同组标本中ITGαvβ3、基质金属蛋白酶(MMP)-9、MMP-13表达水平。结果qRT-PCR、Western印迹检测方式结果均显示ITGαvβ3、MMP-9、MMP-13的表达水平在膝关节炎关节软骨中存在差异性表达,并且随着关节软骨退变程度增加其表达水平明显升高,组间相比有统计学意义(P<0.05);相关性分析发现,ITGαvβ3蛋白表达水平与MMP-9蛋白表达水平呈显著正相关(r=0.7818,P<0.001),ITGαvβ3蛋白表达水平与MMP-13蛋白表达水平呈显著正相关(r=0.7191,P<0.001);MMP-9蛋白表达水平与MMP-13蛋白表达水平之间呈正相关(r=0.8475,P<0.001),并且三者表达水平均与退变程度呈显著正相关(r=0.8482,P<0.001;r=0.9315,P<0.001;r=0.9092,P<0.001)。结论老年膝骨关节炎退变程度越高,其关节软骨中ITGαvβ3、MMP-9、MMP-13的表达水平越高。
Objective To study correlation between expression level of integrin(ITG)αvβ3 in articular cartilage and degeneration degree of elderly knee osteoarthritis(KOA).Methods A retrospective analysis was performed on 71 articular cartilage specimens with different degeneration degree which were obtained from 20 elderly KOA patients who underwent total knee arthroplasty(TKA)and were admitted to the hospital from January 2016 to December 2018.According to Bobinac staging and Mankin score after HE staining,they were divided into normal group(n=5),mild degeneration group(n=23),moderate degeneration group(n=24)and severe degeneration group(n=19).The qRT-PCR and Western blot were applied to detect expression levels of ITGαvβ3,matrix metalloproteinase(MMP)-9 and MMP-13 in different groups.Results The results of qRT-PCR and Western blot showed that there were differences in expression levels of ITGαvβ3,MMP-9 and MMP-13 among KOA articular cartilage.And their expression levels were increased with degeneration degree of articular cartilage increasing(P<0.05).Correlation analysis found that expression level of ITGαvβ3 protein was positively correlated with expression level of MMP-9 protein(r=0.7818,P<0.001),and positively correlated with expression level of MMP-13 protein(r=0.7191,P<0.001).There was positive correlation between expression level of MMP-9 protein and MMP-13 protein(r=0.8475,P<0.001).And expression levels of the three were positively correlated with degeneration degree(r=0.8482,P<0.001;r=0.9315,P<0.001;r=0.9092,P<0.001).Conclusions The higher the degeneration degree of elderly KOA,the higher the expression levels of ITGαvβ3,MMP-9 and MMP-13 in articular cartilage.
