颈段食管癌放射治疗应用个体化发泡剂定位的误差分析研究 被引量:3

摘要 目的:分析在颈段食管癌放射治疗中个体化发泡胶和热塑定位的摆位误差。方法:选取在医院治疗的38例颈段食管癌患者,根据体位固定方法将其分为发泡胶组(18例)和标准头枕组(20例),发泡胶组共60次摆位误差数据,标准头枕组共56次摆位误差。两组患者均在大孔径CT下进行定位扫描,治疗中每周采用电子射野影像装置(EPID)进行摆位误差值测定,并对两种固定方法的摆位误差进行统计分析。结果:在X轴方向的摆位误差中,标准头枕组为(1.87±1.46)mm,发泡胶组为(0.62±0.59)mm,两组比较差异有统计学意义(t=8.36,P<0.05);Y轴方向的摆位误差中,标准头枕组为(1.69±1.34)mm,发泡胶组为(1.50±0.68)mm,两组比较差异无统计学意义;Z轴方向的摆位误差中,标准头枕组为(1.55±1.23)mm,发泡胶组为(1.84±1.60)mm,两组比较差异无统计学意义。结论:颈段食管癌采用个体化发泡胶定位较标准头枕定位在X轴方向的误差更小,其体位固定方法具有临床应用价值。 Objective:To analyze the setup errors of individual polyurethane foam sealing agent and thermoplastic fixation in radiotherapy for esophageal on cervical segment.Methods:38 patients with cervical esophageal cancer who received treatment in hospital were divided into two groups:18 patients in the foam sealing agent group with 60 times of setup errors,and 20 patients in the standard headrest group with 56 times of setup errors.Patients in both groups underwent localization scanning under computed tomography(CT)large-aperture.Electronic portal imaging device(EPID)was adopted for measuring setup error value in each week of treatment.And then the setup errors of the two fixed methods were further analyzed.Results:In the setup error on X-axis direction,it was 1.87±1.46 mm in standard headrest group and 0.62±0.59mm in foam sealing agent group,respectively,and the difference of setup error between the two groups was statistical significant(t=8.36,P<0.05).In the setup error on Y-axis direction,it was 1.69±1.34 mm in standard headrest group and 1.50±0.68mm in foam sealing agent group,respectively,and the difference of that between the two groups was no statistical significant.And in the setup error on Z-axis,it was 1.55±1.23 mm in standard headrest group and 1.84±1.60 mm in foam sealing agent group,respectively,and there was no statistical difference between the two groups.Conclusion:The error of individual polyurethane foam sealing agent fixation is less than that of standard headrest fixation on the X-axis direction in cervical esophageal cancer,and the corresponding position fixation method has the value of clinical application.
