摘要
目的:研究戴明循环(PDCA)管理法在缩短急救生命支持设备调配时长中的作用。方法:按照PDCA研究方法,以除颤仪为研究样本,采用"头脑风暴"法从人员、环境、设备和制度4个维度分析除颤仪调配时间过长的原因,并绘制鱼骨图;针对原因修订调配流程和应急预案完善调配方案,统一标准作业程序对不规范行为进行针对性干预。结果:设计了急救生命支持设备调用覆盖图,制定出急救生命支持设备租用管理办法,统一制作了除颤仪标准作业程序(SOP)标识牌。按PDCA法增进实施干预后,除颤仪的调配时长由2016年平均7 min降为2018年平均3 min45 s。结论:PDCA法能够有效缩短急救生命支持设备的调配时长,提升临床医护人员对急救生命支持设备的熟悉程度,完善医院应急管理制度及流程,可为患者的抢救争取时间。
Objective:To study the effects of Deming cycle(plan,do,check,active,PDCA)in shortening the duration of deployment of life-support equipment in rescue.Methods:According to the research method of PDCA,the defibrillator was used as an example,and the brainstorming method was used to analyze the reasons of deployment duration of the defibrillator from 4 dimensions included personnel,environment,equipment and system.And then fish bone diagram was drawn.Aimed at the reason,the deployment process and emergent pre-arranged plan were revised to improve the deployment scheme,and the standard operation procedure was unified so as to implement targeted intervention for disorderly behavior.Results:A deployment coverage diagram of first-aid life-support equipment was designed,and a rental management method of first-aid life-support equipment was drafted,and a corresponding signboard of standard operating procedure(SOP)of defibrillator was uniformly formulated.After the intervention was implemented according to research results,the deployment duration of defibrillator was reduced from average 7 min in 2016 year to 3 min 45 s in 2018 year.Conclusion:PDCA can effectively shorten the deployment duration of first-aid life-support equipment,and enhance the degree of familiarity of clinical paramedic for first-aid life-support equipment,and improve emergency management system and process of hospital,and it can save time in rescuing patients.
