下肢肌力结合平衡训练对老年糖尿病周围神经病变病人步态调控的影响

Effects of lower limb muscle strength combined with balance training on gait control in elderly patients with diabetic peripheral neuropathy

[目的]评价下肢肌力结合平衡训练对老年糖尿病周围神经病变病人步态调控的影响。[方法]选取上海市2家社区老年糖尿病周围神经病变病人72例,按所属社区分为干预组和对照组,各36例。对照组病人接受常规治疗及护理,干预组病人接受3个月的下肢肌力结合平衡训练。比较两组病人干预前后的步态调控情况。[结果]干预后,干预组病人的足底压力中心轨迹曲线异常率、全足压力变化曲线异常率低于对照组(P<0.05);干预组病人的全足平衡性曲线异常率与对照组比较,差异无统计学意义(P>0.05)。[结论]下肢肌力结合平衡训练对老年糖尿病周围神经病变病人的步态调控存在积极影响,可纠正人体异常的行走步态,增加姿势稳定性。

Objectives:To evaluate the effects of lower limb muscle strength combined with balance training on the gait control in elderly patients with diabetic peripheral neuropathy.Methods:A total of 72 elderly patients with diabetic peripheral neuropathy in two communities in Shanghai were selected.They were divided into the intervention group(n=36)and control group(n=36)according to their own communities.The patients in intervention group were given three months lower limb muscle strength combined with balance training.The patients in control group were given routine treatment and nursing.The status of gait control between two groups was measured before and after intervention.Results:After the intervention,the abnormal proportions of Centre of Pressure(COP)and total force line of the intervention group were significantly lower than those of the control group(P<0.05).The abnormal proportions of full balance line between the intervention group and control group were not significanly different(P>0.05).Conclusions:The lower limb muscle strength combined with balance training has a positive effect on the gait control of elderly patients with diabetic peripheral neuropathy,which can correct the abnormal gait of human body,and increase the stability of posture.
