多元院后管理在脑梗死患者护理服务提升中的应用

Application of Multiple Post-hospital Management in Improving Nursing Service for Patients with Cerebral Infarction

选择某三甲医院2016年7月至2018年6月期间出院的2000名脑梗死患者为研究对象,其中2017年7月至2018年6月出院的脑梗死患者980名为试验组,2016年7月至2017年6月出院的脑梗死患者1020名为对照组,试验组开展多元院后管理,包括专病专访、互联网+慢病管理、居家护理等,对照组进行常规随访。研究发现多元院后管理可降低脑梗死患者半年内再入院率,提高患者对护理服务的满意度及对医院的推荐度,使患者和医院双受益。

2000 patients with cerebral infarction discharged from a Grade Ⅲ-A hospital were selected as the study objects from July 2016 to June 2018. 980 patients discharged from July 2017 to June 2018 were assigned to the trial group, while 1020 patients discharged from July 2016 to June 2017 were assigned to the control group. The patients in the trial group were given multiple post-hospital management, including exclusive interview, Internet plus chronic disease management, home care, etc. The patients in the control group were given routine follow-up. The study found that multiple post hospital management can reduce the readmission rate of patients with cerebral infarction within half a year and it can also improve the satisfaction of nursing services and the recommendation of hospitals. Patients and the hospital can benefit from it.
