河源市学龄前儿童小细胞低色素性贫血调查

摘要 目的分析河源市学龄前儿童的小细胞低色素贫血发生率和其病因构成比,为制定有关预防及治疗措施提供参考依据。方法随机抽取2019年1月~2019年12月在本院参与儿保科健康体检或幼儿园体检的本市学龄前儿童共5000例进行研究,所有入选的筛查对象均进行血常规检查,对筛查结果为小细胞低色素症者,进行血常规、网织红细胞、血清铁蛋白、地贫筛查二项等检查以明确病因,并对其中部分儿童进行异常血红蛋白基因检测。结果5000例学龄前儿童中,检出693例存在小细胞低色素性贫血,发生率为13.86%。693例患儿中,检出地中海贫血509例,占73.44%;缺铁性贫血377例,占54.40%;地中海贫血伴缺铁性贫血193例,占27.84%。结论河源市学龄前儿童出现小细胞低色素贫血特别是单纯小细胞低色素症的一个重要原因为地中海贫血,其次是缺铁性贫血与地中海贫血伴缺铁性贫血,且观察到缺铁性贫血个体存在较高地贫基因检出率,需要临床及社会提高重视程度。 Objective To analyze the incidences and etiological constituent ratio of microcytic hypochromic anaemia of preschool children in Heyuan city,providing basis on clinical prevention and treatment.Methods 5000 preschool children checked in our child health care department or in the kindergartens(2019.01-2019.12)were randomly chosen.Blood routine examination was performed for screening of microcytic hypochromia.In order to determine the specific etiological factors,children with microcytic hypochromia were re-examined for blood routine,reticulocyte,serum ferritin,and thalassemia.The abnormal hemoglobin examination was taken for some children.Results Among 5000 preschool children,693 cases had microcytic hypochromic anaemia(13.86%).509 children had thalassemia(73.44%),377 cases with iron deficiency(54.40%)and 193 thalassemia cases with iron deficiency(27.84%).Conclusion The thalassemia is a key cause of children’s microcytic hypochromic anaemia,especially for microcytic hypochromia,followed by iron deficiency,thalassemia complicated with iron deficiency.It has been found that iron deficiency children are prone to have high detection rate of thalassemia genes.The clinical and social attentions shall be improved.
