祛湿通脉汤湿敷联合曲张静脉抽剥术治疗下肢淤积性皮炎疗效及对溃疡面积、小腿周径和生活质量的影响

Therapeutic effect of Qushi Tongmai decoction by wet compress combined with varicose vein stripping on lower extremity stasis dermatitis and its effect on ulcer area,calf circumference and quality of life

目的探讨祛湿通脉汤湿敷联合曲张静脉抽剥术治疗下肢淤积性皮炎的临床疗效及对溃疡面积、小腿周径和生活质量的影响。方法将2017年6月—2018年6月在石家庄市中医院诊治的100例下肢淤积性皮炎患者随机分为观察组和对照组,每组50例。2组患者均进行曲张静脉抽剥术治疗,观察组患者在此治疗基础上联合祛湿通脉汤湿敷治疗2周。观察2组患者治疗前后溃疡面积、疼痛评分(VAS指数)、小腿周径和生活质量(生理职能、社会职能、躯体功能、总体健康)情况,统计2组临床治疗效果。结果2组治疗后溃疡面积、小腿周径差、VAS评分均较治疗前明显降低(P均<0.05),且观察组明显低于对照组(P均<0.05);2组治疗后生活质量各项评分均较治疗前明显升高(P均<0.05),且观察组明显高于对照组(P<0.05);观察组临床总有效率为92%(46/50),显著高于对照组的72%(36/50),差异有统计学意义(P<0.05)。结论祛湿通脉汤湿敷联合曲张静脉抽剥术可明显促进下肢淤积性皮炎患者恢复,改善生活质量,治疗效果明显优于单用曲张静脉抽剥术。

Objective It is to explore the clinical effect of Qushi Tongmai decoction by wet compress combined with varicose vein stripping on lower extremity stasis dermatitis and its effect on ulcer area,calf circumference and quality of life.Methods 100 patients with lower extremity stasis dermatitis diagnosed and treated at Shijiazhuang Hospital of Traditional Chinese Medicine from June 2017 to June 2018 were randomly divided into observation group and control group,50 cases in each group.The patients in both groups were treated with varicose vein stripping,and the patients in the observation group were treated with Qushi Tongmai decoction by wet compress for 2 weeks.The ulcer area,pain score(VAS index),calf circumference and quality of life(physiological function,social function,physical function,overall health)of the patients in both groups were observed before and after treatment,and the clinical curative effect of the 2 groups were assessed.Results After treatment,the ulcer area,calf circumference difference,and VAS score of the two groups were significantly lower than those before treatment in the two groups(all P<0.05),and the indexes in the observation group were significantly lower than thsoe in the control group(P<0.05);the scores of quality of life of the two groups after treatment were significantly higher than those before treatment(P<0.05),and the scores in the observation group were significantly higher than those in the control group(P<0.05);the total clinical effective rate of the observation group was 92.00%(46/50),which was significantly higher than 72.00%(36/50)of the control group,the difference was significant(P<0.05).Conclusion Qushi Tongmai decoction by wet compress combined with varicose vein stripping can obviously promote the recovery of patients with lower extremity stasis dermatitis and improve the quality of life.The treatment effect is significantly better than varicose vein stripping alone.
