摘要
目的探讨超声造影(contrast-enhanced ultrasound,CEUS)在乳腺癌病灶增强后扩大程度与其影响因素。方法选取我院2017年2月~2018年10月乳腺癌患者78例为观察组,另选我院乳腺良性肿块患者80例作为对照组,对两组患者术后标本免疫组化指标表达进行观察,并对免疫组化指标与CEUS增强后区域扩大的相关性进行分析。结果观察组CEUS前后最大横径、最大纵径对比差异具有统计学意义(P<0.05)。观察组与对照组CEUS后增强效果、增强类型、造影剂分布特征对比,差异具有统计学意义(P<0.05)。观察组ER、PR、HER-2、Ki67阳性表达占比率显著高于对照组,阴性表达占比率显著低于对照组,差异具有统计学意义(P<0.05)。Pearson相关性分析显示造影后最大横径、最大纵径与ER、PR、HER-2、Ki67表达之间相关性均为正相关关系,差异具有统计学意义(P<0.05)。结论乳腺癌CEUS检查其部分特征与乳腺良性肿块存在差异,其增强区域扩大的程度与免疫组化指标之间存在正相关关系。
Objective To study the expansion degree and its influencing factors of contrast-enhanced ultrasound in breast cancer.Methods From February 2017 to October 2018,78 breast cancer patients in our hospital were selected as the observation group,and 80 breast benign tumor patients in our hospital were selected as the control group.The expressions of immunohistochemistry indexes in the samples of the two groups were observed,and the correlation between the immunohistochemistry indexes and the regional expansion after CEUS enhancement was analyzed.Results The difference of the maximum transverse diameter and the maximum longitudinal diameter before and after CEUS in the observation group was statistically significant(P<0.05).The difference of CEUS enhancement effect,enhancement type and contrast agent distribution between the observation group and the control group was statistically significant(P<0.05).The positive expression rate of Er,PR,HER-2 and Ki67 in the observation group was significantly higher than that in the control group,while,the negative expression rate was significantly lower than that in the control group(P<0.05).Pearson correlation analysis showed that the correlation between the maximum transverse diameter and the maximum longitudinal diameter and the expression of Er,PR,HER-2 and Ki67 was positive,the difference was statistically significant(P<0.05).Conclusion Some features of CEUS in breast cancer were different from those of benign breast masses,and there was a positive correlation between the extent of enhancement area and immunohistochemistry.
