摘要 [目的]探讨无线体温传感器iThermonitor■在手术病人核心体温监测中的应用效果。[方法]选取2018年6月—12月胸外科、神经外科手术病人226例,术前在病人腋窝处粘贴无线体温传感器iThermonitor■监测病人的核心体温,记录手术病人低体温发生率、围术期体温变化及相关因素、术后住院时间及医疗费用。[结果]胸外科手术病人低体温发生率为70.83%,体温跌幅范围为0.008~2.883℃;神经外科手术病人低体温发生率为73.59%,体温跌幅范围为0.007~2.789℃。回归分析显示,手术时长、体质指数(BMI)、年龄是胸外科手术病人体温的影响因素;手术时长、BMI是神经外科手术病人体温的影响因素。胸外科手术病人发生低体温,术后住院时间与费用增高;神经外科手术病人发生低体温,术后住院时间与费用减少。[结论]手术室护理人员应根据手术病人情况,采用智能化监测方法进行全程链式体温管理,保证手术病人的安全。 Objective:To probe into the application effect of wireless body temperature sensor iThermonitor■in monitoring core body temperature of intraoperative patients.Methods:A total of 226 cases of patients who were admitted in two departments of thoracic surgery and neurosurgery from June to December 2018,were selected as subjects.Before the operation,a wireless temperature sensor iThermonitor■was adhered to the armpit of the patients,in order to monitor the patient's core body temperature.The incidence of hypothermia,changes of perioperative temperature and its related factors,postoperative hospitalization duration and medical expenses were recorded.Results:The incidence of hypothermia in patients undergoing thoracic surgery was 70.83%.The drop degree of body temperature ranged from 0.008 to 2.883℃.The incidence of hypothermia in patients undergoing neurosurgery was 73.59%.The drop degree of body temperature ranged from 0.007 to 2.789℃.Regression analysis showed that duration of surgery,body mass index(BMI)and age were the influencing factors of body temperature of patients undergoing thoracic surgery.Length of operation and BMI were the influencing factors of body temperature of patients undergoing neurosurgery.Hypothermia in patients undergoing thoracic surgery resulted in the increase of hospitalization duration and cost.Hypothermia in patients undergoing neurosurgery resulted in the decrease of hospitalization duration and cost.Conclusion:Based on the characteristics of surgical patients,nurses in operating room should adopt intelligent monitoring method to conduct whole⁃process chain temperature management,thus to ensure the safety of patients.
