基于Web of Science及CNKI数据库预立医疗照护计划中美研究态势与热点领域对比

Comparisons of research trends and hot areas between China and America in advance care planning based on Web of Science and CNKI database

[目的]应用知识图谱技术探究中美预立医疗照护计划研究发展趋势异同及中国自身特点。[方法]通过Web of Science(WOS)以及中国知网(CNKI)数据库检索2001年—2018年发表的预立医疗照护计划相关文献,利用科学引文分析及可视化软件Citespace进行文献计量学分析。[结果]共纳入文献3187篇,其中来源于WOS数据库3045篇,来源于CNKI数据库142篇。中美预立医疗照护计划领域高影响力期刊发文占比存在显著差异。美国预立医疗照护计划的主要研究领域为卫生保健、全科医学、内科学和老年医学;中国预立医疗照护计划研究领域主要集中在老年病学与肿瘤学领域。美国预立医疗照护计划领域研究热点主要包含5类:不同人群的预立医疗照护计划、生命维持治疗、预立医疗照护计划相关情感与经历、接受度、干预措施;中国预立医疗照护计划研究领域研究热点主要包含4类:姑息治疗与安宁疗护、生前预嘱、预立医疗照护计划接受度影响因素、认知与态度。[结论]预立医疗照护计划领域研究对象、研究方法、研究主题呈现多样化。中国应加强预立医疗照护计划在医疗领域中应用的过程与效果研究,开发具有中国文化特点的干预模型,促进中国安宁疗护事业的发展。

Objective:Using knowledge mapping technology,to probe into the similarities and differences of the research trends in advance care planning between China and America,and to analyze the Chinese characteristics.Methods:Literatures related to advance care planning were retrieved from Web of Science(WOS)and CNKI database.The retrieval time was from 2001 to 2018.And bibliometrics analysis was conducted by scientific citation analysis and visualization software Citespace.Results:A total of 3187 literatures were included,involving 3045 literatures from WOS and 142 literatures from CNKI.There was difference in the proportion of published articles in high⁃impact journals in the field of advance care planning between China and America.The main research areas of advance care planning in America,were health care,general practice medicine,internal medicine,and geriatrics.The research areas of advance care planning in China,mainly focused on geriatrics and oncology.The research hotspots of advance care planning in America mainly consisted of five categories:different populations of advance directives,life maintenance therapy,advance care planning-related emotions and experiences,acceptability,and intervention measures.The research hotspots of advance directives in China mainly included four categories:palliative treatment and hospice care,living will,factors influencing the acceptability of advance care planning,cognition,and attitude.Conclusions:Research objects,methods and themes in advance care planning are diversified.It is adviced to enhance the researches related to application process and application effect of advance care planning in the medical field in China,create the intervention model charactered with Chinese culture,and promote the development of hospice care in China.
