血管内支架置入术治疗颅外段颈动脉粥样硬化狭窄的临床疗效

Effect of endovascular stent implantation on extracranial carotid atherosclerotic stenosis

目的探讨血管内支架置入术治疗颅外段颈动脉粥样硬化狭窄的临床疗效。方法选取2017年1月至2019年11月在河池市第一人民医院进行治疗的颅外段颈动脉粥样硬化狭窄患者66例,根据治疗方法不同分为观察组和对照组,各33例。统计并比较治疗后两组患者临床疗效、动脉狭窄情况;检测并比较治疗前后血压、生化(血清肌酐和超敏C反应蛋白)及血脂水平[高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)、甘油三酯(TG)]。结果观察组患者总有效率为96.97%高于对照组患者60.61%,并发症发生率为3.03%低于对照组患者24.24%;且观察组患者收缩压、舒展压、血清肌酐、超敏C反应蛋白、HDL-C、LDL-C、TG和TC均优于对照组患者,颅外段颈动脉粥样硬化狭窄患者狭窄程度轻于对照组患者,差异有统计学意义(P<0.05)。结论血管内支架置入术治疗颅外段颈动脉粥样硬化狭窄的效果显著且安全性高,能改善患者预后,值得研究和推广。

Objective To investigate the clinical effect of endovascular stent placement in the treatment of extracranial carotid atherosclerotic stenosis.Method From January 2017 to November 2019,66 patients with extracranial carotid atherosclerotic stenosis who were treated in the first people's Hospital of Hechi were divided into observation group and control group according to different treatment methods,33 cases in each group.The blood pressure,biochemical(serum creatinine and high sensitive C-reactive protein)and blood lipid levels(HDL-C,LDL-C,TC,TG)were measured and compared before and after treatment.Result The total effective rate of the observation group was 96.97%higher than that of the control group(60.61%),and the incidence of complications was 3.03%lower than that of the control group(24.24%);the systolic pressure,diastolic pressure,serum creatinine,high-sensitivity C-reactive protein,HDL-C,LDL-C,TG and TC of the observation group were better than those of the control group,the degree of stenosis in patients with extracranial carotid stenosis was lighter than that in the control group(P<0.05).Conclusion Endovascular stent implantation is effective and safe in the treatment of extracranial carotid atherosclerotic stenosis,which can improve the prognosis of patients,and is worthy of study and promotion.
