B超引导神经内镜微创手术治疗高血压脑出血的疗效研究

Clinical Effect of Ultrasound Guided Neuroendoscopy in Minimally Invasive Surgery of Hypertensive Intracerebral Hemorrhage

目的探讨B超引导神经内镜微创手术治疗高血压脑出血(hypertensive intracerebral hemorrhage,HICH)的临床效果及应用价值。方法选取150例高血压脑出血患者,随机分为内镜组75例,行B超引导神经内镜微创手术;穿刺组75例,行CT定位下经皮微创穿刺引流术。比较两组穿刺到预先设定位置成功率、血肿清除率、术后3个月预后分级和预后优良率。结果与穿刺组相比,内镜组穿刺至预设位置成功率、术后血肿清除率均明显增高(P<0.05),术后3个月内镜组预后优良率为86.66%,明显高于穿刺组的69.33%(P<0.05)。结论B超引导神经内镜微创手术治疗HICH的疗效显著,具有较高的临床应用价值。

Objective To evaluate the application value and the clinical effects of minimally invasive operation guided by B ultrasound in hypertensive intracerebral hemorrhage(HICH).Methods One hundred and fifty cases of HICH patients were randomly divided into two groups,seventy-five cases for each.In the study group,the patients were treated by endoscopic minimally invasive surgery guided by B ultrasound.And in the control group,the patients received percutaneous minimally invasive puncture drainage under CT.The successful puncture rate of preset position,the hematoma clearance rate,and the good prognosis rate at 3 months after operation were compared between the two groups.Results Compared with the control group,the successful puncture rate of preset position and the hematoma clearance rate in the study group was significantly higher(P<0.05).At 3 months after surgery,the good prognosis rate in the study group was significantly higher than that of the control group(86.66%vs.69.33%)(P<0.05).Conclusions It has a significant effect with a high clinical value of minimally invasive endoscopic operation guided by B ultrasound in the treatment of HICH.
