妊娠期高血压疾病产妇孕中、孕晚期血脂水平与围产结局及性生活质量相关性分析 被引量:14

Correlation between blood lipid levels in middle and late pregnancy and perinatal outcomes and quality of sexual life in pregnant women with hypertensive disorder complicating pregnancy

摘要 目的探讨妊娠期高血压疾病(HDCP)产妇孕中、孕晚期血脂水平与围产结局及性生活质量的相关性。方法选取2017年4月至2018年4月深圳市宝安区沙井人民医院诊治的60例HDCP产妇作为研究对象,将这60例产妇设为观察组,另取60例同期分娩的健康产妇设为对照组,统计并比较两组产妇孕中和孕晚期甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、性生活质量及围产结局的差异,并分析血脂水平与围产结局的相关性。结果与对照组相比,观察组孕中、孕晚期TG水平明显较高,孕中和孕晚期TC、HDL水平明显较低,差异具有统计学意义(P<0.05);观察组性生活质量评分明显低于对照组,差异具有统计学意义(P<0.05);观察组早产、剖宫产发生率明显高于对照组,差异具有统计学意义(P<0.05);二元Logistic相关性分析显示,孕中和孕晚期TG、HDL、LDL是HDCP产妇早产的独立正相关因素,孕中和孕晚期TC是HDCP产妇早产的独立负相关因素;其他不良围产结局与孕中和孕晚期血脂指标均无相关性。结论妊娠期高血压疾病可导致血脂代谢异常、产后性生活质量降低,并造成不良围产结局。孕中和孕晚期TG、HDL、LDL是早产的独立危险因素,孕中、孕晚期TC是早产的保护因素。临床应密切监测血脂变化,及时干预,以减少不良围产结局的发生。 Objective To investigate the correlation between serum lipid levels in middle and late pregnancy and perinatal outcomes and quality of sexual life in pregnant women with hypertensive disorder complicating pregnancy(HDCP).Methods 60 HDCP women admitted to Shajing People′s Hospital from April 2017 to April 2018 were selected as observation group,and 60 healthy women who were delivered at the same time were selected as the control group.The levels of triglycerides(TG),total cholesterol(TC),high-density lipoprotein(HDL)and low-density lipoprotein(LDL)in middle and late pregnancy and differences in quality of sexual life and perinatal outcomes were compared between the two groups to analyze the correlation between blood lipid levels and perinatal outcomes.Results Compared with the control group,the TG level in the middle and late pregnancy was significantly higher in the observation group,and the levels of TC and HDL were significantly lower,with statistically significant differences(P<0.05).The quality of sexual life in the observation group was significantly lower than that in the control group,with statistically significant differences(P<0.05).The incidence of preterm labor and cesarean section in the observation group was significantly higher than that in the control group,with statistically significant differences(P<0.05).The logistic correlation analysis showed that TG,HDL and LDL in the middle and late pregnancy were independent positive correlation factors of preterm delivery of HDCP and TC were independent negative correlation factors for preterm delivery of HDCP.There was no correlation between other adverse perinatal outcomes and the levels of blood lipids in the middle and late pregnancy.Conclusions HDCP can lead to abnormal blood lipids,a decrease in the quality of postpartum sexual life,and poor perinatal outcomes.TG,HDL,LDL in middle and late pregnancy are independent risk factors for preterm birth,and TC is a protective factor for preterm birth.Therefore,blood lipid changes should be closely monitored and promptly intervened in clinical to reduce the occurrence of adverse perinatal outcomes.
