两种肠道准备方法在服用阿司匹林肠溶片患者行胶囊内镜检查中的效果比较

Effect comparisons of two methods of bowel preparation in patients taking enteric-coated aspirin undertaking capsule endoscopy

目的比较2种肠道准备方法在服用阿司匹林肠溶片患者行胶囊内镜检查中的效果。方法招募2019年6月至2020年1月在首都医科大学附属北京安贞医院就诊的服用阿司匹林肠溶片患者20例,间隔2个月分别以聚乙二醇电解质散和复方匹克硫酸钠颗粒行肠道准备,比较2次胶囊内镜检查小肠清洁效果和患者肠道准备体验的差别。结果20例患者中男14例、女6例,平均年龄(56±9)岁,其中7例患者无小肠黏膜损伤、8例患者存在小肠黏膜糜烂、5例患者存在小肠黏膜溃疡。2次肠道准备方法小肠通过时间,小肠近端、中段、远端清洁度评分,小肠近端、中段、远端气泡评分差异均无统计学意义(均P>0.05)。应用复方匹克硫酸钠颗粒时口味满意度、肠道准备体验、药液更容易服用得分高于应用复方聚乙二醇电解质散时[(8.2±1.2)分比(5.9±1.8)分、(8.0±1.2)分比(6.6±1.4)分、(8.4±1.1)分比(6.6±1.6)分],差异均有统计学意义(均P<0.001)。不同小肠损伤程度患者,2种肠道准备方法在口味满意度、肠道准备体验和药液更容易服用评分方面差异均无统计学意义(均P>0.05)。结论服用阿司匹林肠溶片患者存在不同程度的小肠黏膜损伤,但不影响患者的肠道准备体验。聚乙二醇电解质散和复方匹克硫酸钠均可用于正在服用阿司匹林肠溶片患者胶囊内镜检查的肠道准备,且2种方法之间小肠清洁程度无明显差别,但复方匹克硫酸钠行肠道准备更容易被患者接受。

Objective To compare the effect of two methods of bowel preparation in patients taking enteric-coated aspirin undertaking capsule endoscopy.Methods From June 2019 to January 2020,20 patients taking enteric-coated aspirin admitted in Beijing Anzhen Hospital,Capital Medical University were selected.They were given capsule endoscopy with bowel preparation of polyethylene glycol electrolyte powder and compound sodium picosulfate/magnesium citrate at intervals of 2 months.The difference between the cleaning effect of the small intestine with the capsule endoscopy and the patient's intestinal preparation experience were compared.Results There were 14 males,6 females.Average age was(56±9)years old,7 cases showed no small intestinal mucosal injury,8 cases suffered small intestinal mucosal injury of erosion,and 5 cases suffered small intestinal mucosal injury of ulceration.There were no significant difference on the small intestinal transit time,cleanliness/bubble scores of the proximal,middle and distal segments of the small intestine between the two methods(all P>0.05).The score of taste satisfaction,bowel preparation experience and the ease of drug administration of compound sodium picosulfate/magnesium citrate were higher than those of compound polyethylene glycol electrolyte powder[(8.2±1.2)vs(5.9±1.8),(8.0±1.2)vs(6.6±1.4),(8.4±1.1)vs(6.6±1.6),all P<0.001].There was no significant difference in the taste satisfaction,bowel preparation experience and easy of drug administration between the two bowel preparation methods in patients with different degree of small intestine injury(all P>0.05).Conclusions There were different degrees of small intestinal mucosal damage in patients taken enteric-coated aspirin,but it didn't affect the bowel preparation experience.Polyethylene glycol electrolyte powder and compound sodium picosulfate/magnesium citrate can both be used for bowel preparation for capsule endoscopy in patients taking enteric-coated aspirin,but compound sodium picosulfate/magnesium citrate for bowel preparation was more easily accepted.
