摘要
目的 探讨16排螺旋CT增强扫描在诊断原发性肝癌应用的对比剂剂量及流速.方法 以2016年5月-2019年7月期间孟津县人民医院肿瘤科收治的86例原发性肝癌患者为研究对象,按照随机数表法分为对照组与观察组,各43例.平扫之后设置延迟时间为0.5min、1.0min、3.0min;注射对比剂剂量:对照组为80~90mL,观察组为110~120mL,可酌情适当增减剂量,采集患者不同时期的扫描图像并分析其特征.结果 经16排螺旋CT增强扫描分析,观察组检出病灶59个,对照组为58个.与对照组相比,观察组动脉期检出率较高(P<0.05);门脉期及平衡期检出率方面观察组患者均略高于对照组,但差异均无统计学意义(P>0.05);观察组患者对比剂剂量高于对照组,观察组患者CT图像特征质量明显高于对照组,在平衡期与门脉期扫描强化优于对照组.结论 16排螺旋CT增强扫描在诊断原发性肝癌中,较高的剂量及速度能够有助于更加清晰的显示病灶及血供情况,利于指导临床治疗.
Objective To evaluate the contrast dose and flow rate of 16-slice spiral CT in the diagnosis of primary liver cancer.Methods From May 2016 to July 2019,86 patients with primary liver cancer admitted to the Oncology Department of Mengjin County People's Hospital were divided into control group(43 cases)and Observation Group(43 cases)according to random number table method.The delay time was set to 0.5 min,1.0 min and 3.0 min after plain scan,and the contrast dose was 80-90 ML in control group and 110-120 ML in observation group,the appropriate dose can be appropriate to increase or decrease,the acquisition of patients at different stages of scanning images are collected and analyzed its characteristics.Results 59 lesions were detected in the observation group and 58 lesions in the control group after16-slice spiral CT enhanced scanning analysis.Compared with the control group,the detection rate of arterial phase in the observation group was higher(P<0.05),and the detection rate of portal phase and equilibrium phase in the observation group were slightly higher than the control group,but the difference was not statistically significant(P>0.05);The dose of contrast medium in Observation Group was higher than that in control group,the quality of CT image in observation group was better than that in control group,and the scanning enhancement in the balance period and portal phase was better than that in the control group.Conclusion In the diagnosis of primary liver cancer,16-slice spiral CT enhanced scan,the higher dose and speed can help to show the focus and blood supply more clearly,in order to guide clinical treatment.
