改良呼出气冷凝液收集装置和EcoScreen冷凝装置用于全身麻醉机械通气患者呼出气冷凝液收集的比较 被引量:1

摘要 目的:明确改良呼出气冷凝液(exhaled breath condensate,EBC)收集装置和EcoScreen冷凝装置用于全身麻醉机械通气患者EBC标本收集的效果并进行比较。方法:选择拟择期在全麻下行开腹手术的患者60例,随机分为改良组(M组)和EcoScreen组(E组),每组30例。两组于手术开始后10 min串联入EBC收集装置收集EBC标本,收集时间为20 min。记录患者手术开始时(T1)、手术开始后10 min(T2)、20 min(T3)、30 min(T4)、40 min(T5)和50 min(T6)时的生命体征、潮气量(tidal volume,Vt)、气道峰压(peak airway pressure,Ppeak)、气道平均压(mean airway pressure,Pmean)等指标。测量所收集的EBC量,并对EBC中的8-异前列腺素F2α(8-iso-prostaglandin F2α,8-iso-PGF2α)质量浓度进行测定,同时观察患者术后气道相关不良反应的发生情况。结果:两组患者不同时间点生命体征、Vt、Ppeak、Pmean等差异均无统计学意义(均P>0.05)。两组患者术后气道相关不良反应的发生率差异均无统计学意义(均P>0.05)。M组EBC收集量[(1.05±0.37)mL]低于E组[(1.99±0.53)mL](P<0.05),但两组患者EBC中8-iso-PGF2α的质量浓度差异无统计学意义(P>0.05)。结论:在全身麻醉机械通气患者EBC标本收集过程中,改良EBC收集装置收集效率低于EcoScreen冷凝装置,但其收集成本低,可满足短时间内多次取样检测。 Objective:To compare the relative merits between modified exhaled breath condensate(EBC)collection device and EcoScreen condensate device used for the collection of EBC in patients undergoing mechanical ventilation during anesthesia.Methods:A total of sixty patients receiving laparotomy under general anesthesia were randomly divided into two groups(n=30):modified group(group M),EcoScreen condenser group(group E).EBC specimens of the two groups were collected by in tandem with the EBC collection device 10 min after the beginning of operation,and the collection time was 20 min.Vital signs,tidal volume(Vt),peak airway pressure(Ppeak),and mean airway pressure(Pmean)were examined at the beginning of surgery(T1),10 min(T2),20 min(T3),30 min(T4),40 min(T5),50 min(T6)after the beginning of surgery.EBC amount and concentration of 8-iso-prostaglandin F2α(8-iso-PGF2α)were measured.Postoperative airway-related adverse reactions were also assessed.Results:There were no significant differences in vital signs,Vt,Ppeak,Pmean of different time points and incidence of airway-related adverse reactions between the two groups(P>0.05).Amount of EBC in group M[(1.05±0.37)mL]was lower than group E[(1.99±0.53)mL](P<0.05).However,the concentration of 8-iso-PGF2αwas no significant difference between the two groups(P>0.05).Conclusion:Modified EBC collection device has a lower efficiency than EcoScreen condensate device in the collection of EBC in patients undergoing mechanical ventilation during anesthesia,but it relative to low cost and repeatable collection in a short period of time.
