摘要
目的探讨胫腓骨骨折患者内固定术后Buerger运动锻炼对患肢血流变化的影响。方法选取胫腓骨骨折切开复位内固定术患者60例作为研究对象,随机分为对照组和观察组各30例。在常规护理基础上,两组患者均于术后第1天开始行患肢运动锻炼,观察组行Buerger运动,对照组行常规运动,比较两组患者干预前及1、3、5、7 d干预结束后患肢足背动脉血流动力学参数收缩期峰值血流速度(FS)、舒张期峰值血流速度(FD)、最小血流速度(FM)、平均血流速度(TAM)、搏动指数(PI)、阻力指数(RI)的变化,以及患肢疼痛(VAS)与肿胀情况。结果随着干预时间的延长,两组患者足背动脉各血流动力学参数水平、患肢疼痛评分、周径差值比较差异均有统计学意义(P<0.05)。结论术后行Buerger运动锻炼能够增加胫腓骨骨折患者内固定术后患肢的血流,减轻患肢疼痛及肿胀。
Objective To investigate the effect of Buerger’s exercise on blood flow changes of the affected limb after internal fixation in patients with tibiofibular fractures.Methods A total of 60 patients with tibiofibular fractures treated by open reduction and internal fixation was chosen as participants.This study was randomly divided into the control group(n=30)and the observation group(n=30).On the basis of routine nursing,both groups began to exercise the affected limbs on the first day after operation.Then,routine exercise was performed in the control group and Buerger’s exercise in the observation group.To compare the hemodynamic parameters of limb dorsal artery in the two groups before intervention and after the intervention at 1,3,5 and 7 days,the parameters collected were:Systolic Flow(FS),Minimum Flow(FM),Time Average Maximum(TAM),Pulsatility Index(PI),Resistance Index(RI),Visual Analogy Score(VAS score)and Swelling.Results With the prolongation of the intervention time,there were statistically significant differences in the hemodynamic parameters,limb pain score and difference of circumference between the two groups.Conclusion Functional exercises after internal fixation in patients with tibiofibular fractures could accelerate the blood flow,reduce the pain and swelling of the affected limbs and promote fracture healing.
