老年宫颈癌患者根治术后尿管留置时间对其预后的影响

Effect of urinary indwelling time on prognosis of elderly patients with cervical cancer after radical operation

目的探讨老年宫颈癌患者行根治术后尿管留置时间的长短对其预后的影响。方法回顾性分析82例老年宫颈癌患者的临床资料,根据术后尿管留置时间的长短,将其分为2周组(留置2周,42例)、3周组(留置3周,40例),并对两组患者的基本资料,拔除尿管首次排尿后的膀胱残余尿量、膀胱功能分级,留置尿管并发症进行统计学分析。结果两组患者的年龄、BMI值、腹部手术史次数无统计学差异(P>0.05);3周组患者的膀胱残余尿量低于2周组、膀胱功能优于2周组(P均<0.05);两组患者尿路感染及尿潴留的风险差异无统计学意义(P>0.05);与2周组相比,3周组患者二次留置尿管的风险降低(P均<0.05)。结论老年宫颈癌患者行根治术后,适当延长尿管留置时间,可在不增加尿路感染及尿潴留风险的前提下降低二次留置尿管的风险,改善患者预后。

Objective To investigate the effect of urinary indwelling time on the prognosis of elderly patients with cervical cancer after radical operation.Methods The clinical data of 82 elderly patients with cervical cancer were retrospectively analyzed.According to the length of postoperative urinary indwelling time,patients were divided into 2 weeks group(2 weeks indwelling,42 cases)and 3 weeks group(3 weeks indwelling,40 cases).The basic data of the two groups of patients,the residual urine volume of the bladder after the first urination,the bladder function classification,and the complications of the indwelling catheter were statistically analyzed.Results There was no significant difference in age,BMI value,and number of abdominal surgery history between the two groups.The residual urine volume of bladder in 3 weeks group was lower than that in 2 weeks group,and the bladder function was better than that in 2 weeks group.There was no significant difference in the risk of urinary tract infection and urinary retention between the two groups.Compared with 2 weeks group,the risk of secondary indwelling catheters in group 3 weeks was reduced.Conclusion Prolonged urinary indwelling time in elderly patients with cervical cancer could reduce the risk of secondary indwelling without increasing the risk of urinary tract infection and urinary retention,and further improve the prognosis of patients.
