通淋汤保留灌肠联合体外高频热疗治疗Ⅲ型慢性前列腺炎的临床疗效研究

Clinical efficacy of external high frequency thermotherapy plus retention enema with the Tonglin decoction on typeⅢchronic prostatitis

目的:评价通淋汤保留灌肠联合体外高频热疗治疗Ⅲ型慢性前列腺炎的临床疗效,为临床治疗用药提供参考。方法:筛选98例Ⅲ型慢性前列腺炎,按治疗方式不同随机分为治疗组49例、对照组49例,对照组给予左氧氟沙星胶囊治疗;治疗组给予通淋汤保留灌肠联合体外高频热疗,连续治疗14天,治疗后随访6个月。结果:两组患者治疗后在疼痛、排尿症状、生活质量方面,分值均显著低于治疗前,P均<0.05,差异有统计学意义。两组患者治疗后在疼痛、排尿症状、生活质量方面比较,P均<0.05,差异有统计学意义。根据中医辨证评分两组患者在尿路症状、放射痛症状、全身症状方面,分值均显著低于治疗前,治疗前后比较,P均<0.05,差异有统计学意义。治疗组的中医证候评分较对照组下降更明显,P均<0.05,差异有统计学意义。两组患者治疗前后中医证候疗效比较,治疗组优于对照组,治疗组中显效19例,有效27例,无效3例,总有效率为93.9%(46/49),对照组中显效14例,有效26例,无效9例,总有效率为81.6%(40/49),两组比较,P<0.05,差异有统计学意义。两组患者治疗后随访6个月,治疗组复发例数8例,对照组复发例数18例,治疗组复发率明显低于对照组,两组比较差异有统计学意义(P<0.05)。结论:通淋汤保留灌肠联合体外高频热疗可显著降低慢性前列腺炎患者的前列腺炎症状评分,中医证候评分,可以有效缓解患者临床症状,提高患者生活质量,且复发率低,安全性高。

Objective:To evaluate the clinical efficacy of retention enema with the Tonglin decoction(通淋汤)plus external high frequency thermotherapy on type Ⅲ chronic prostatitis,and to provide reference for clinical treatment.Methods:98 cases of type Ⅲ chronic prostatitis were selected and randomly divided into the control group and the observation group,49 cases in each.The control group was treated with levofloxacin capsules,and the observation group was treated with the Tonglin decoction retention enema plus external high frequency thermotherapy.A course of treatment was 7 days.Both groups were treated for 2 courses.Followed up for 6 months after treatment.Results:The scores of pain,urinary symptoms and quality of life in the two groups were significantly lower than those before treatment(P<0.05).Pain,urinary symptoms and quality of life were significantly lower in the two groups after treatment,P<0.05.Among the 49 patients in the treatment group,19 cases were markedly effective,27 cases were effective,3 cases were ineffective,and the total efficiency was 93.9%(46/49).Among the 49 patients in the control group,14 cases were markedly effective,26 cases were effective,9 cases were ineffective,and the total efficiency was 81.6%(40/49).The patients in the two groups were followed up for 6 months after treatment.There were 8 cases of recurrence in the treatment group and 18 cases in the control group.The recurrence rate in the treatment group was significantly lower than that in the control group.The difference between the two groups was statistically significant(P<0.05).Conclusion:The Tonglin decoction retention enema plus external high frequency thermotherapy can significantly reduce the NIH-CPSI scoreand the TCM syndrome score,improve the quality of life of patients,and has low recurrence rate and high safety.
