胃溃疡应用四君子汤加味治疗的效果观察

Effect observation of Sijunzi decoction on gastric ulcer

目的:分析应用四君子汤加味治疗胃溃疡的效果.方法:选取高州市中医院2016年的5月-2018年的3月所诊治的96例胃溃疡患者,按照随机方法分为治疗组和对照组,包括遵医嘱应用兰索拉唑、克拉霉素、阿莫西林等药物施治.治疗组则选用四君子汤加味施治,方组内药物:生甘草3 g,延胡索10 g,白芍10 g,木香10 g,陈皮10 g,白扁豆10 g,芡实10 g,党参片10 g,白术10 g,茯苓10 g,海螵蛸15 g,薏苡仁15 g.属寒邪克胃者加用小茴香6 g,吴茱萸6 g,高良姜10 g;属肝气犯胃者加用青皮、香附、柴胡各10 g;属饮食停滞者加用炒谷芽、炒麦芽、神曲各15g;属胃热积滞者加用苦参10 g,黄芩10 g,马齿苋15 g;属胃气上逆者加用代赭石、旋覆花、法半夏各10 g;属湿浊中阻者加用白豆蔻3 g,佩兰10 g,藿香10 g;属胃阴亏虚者加用干石斛10 g,玉竹15 g,北沙参15 g.观察两组患者接受不同治疗模式后胃溃疡症状的改善情况;行胃镜检查掌握病灶变化.对比两组治疗后的临床疗效.结果:治疗组与对照组比较治疗总有效率更高,两组比较存在明显差异,P<0.05.结论:应用四君子汤加味治疗胃溃疡,符合胃溃疡的病机,能促进患者症状改善、病灶愈合,增进治疗效果,且中药方组具备较高安全性,利于患者耐受,也能提高依从性.

Objective:To analyze the effect of Sijunzi decoction(四君子汤)on gastric ulcer.Methods:Select Gaozhou Chinese Medicine Hospital from May 2016 to March 201896 patients with gastric ulcer were randomly divided into the treatment group and the control group,who received lansoprazole,clarithromycin,amoxicillin and so on.The treatment group was treated with Sijunzi decoction as follows:raw Glycyrrhiza uralensis Fisch 3 g,Corydalis 10 g,Paeonia lactiflora 10 g,Muxiang(木香)10 g,Chen Pei(陈皮)10 g,lentil 10 g,Euryale 10 g,Codonopsis pilosula 10 g,Atractylodes macrocephala 10g,Poria Coix 10 g,squid bone 15 g,Coix seed 15 g.Those with cold evil stomach were treated with fennel 6 g,Evodia officinalis 6 g,Gaoliang ginger(高良姜)10 g;those with liver qi invading stomach were treated with green skin,Xiangfu(香附)and Bupleurum chinense each;those with stagnant diet were treated with fried grain malt,fried malt and Shenqu(神曲)respectively;those with stomach heat stagnation were treated with Sophora flavescens 10 g,Scutellaria baicalensis Georgi 10 g,purslane 15 g;those with stomach qi were added ochre,rotation and turbid flowers,and each of Pinellia ternata was 10 g;those with dampness stagnation were added 10 g of Sophora flavescens,10 g of Scutellaria baicalensis Georgi and 15 g of purslane.The patients with deficiency of stomach yin were treated with Dendrobium candidum(10 g),Phyllostachys pubescens(15 g)and Panax quinquefolium(15 g).The improvement of gastric ulcer symptoms after receiving different treatment modes was observed.Gastroscopy was performed to determine the lesion changes.The clinical curative effect after treatment was compared between the two groups.Results:The total effective rate of the treatment group was higher than that of the control group,and there was significant difference between the two groups(P<0.05).Conclusion:The application of Sijunzi decoction in the treatment of gastric ulcer is in accordance with the pathogenesis of gastric ulcer,which can promote the improvement of symptoms,focus healing and therapeutic effect,and the traditional Chinese medicine group has higher safety,which is beneficial to patient tolerance and can also improves compliance.
