门诊中药房控制调剂内差比较分析

Comparative analysis of control dispensing difference in outpatient Chinese medicine pharmacy

目的:研究影响中药调剂内差的相关因素,控制门诊中药房的调剂内差,加强并改进中药调剂措施管理的实施.方法:通过选取深圳市罗湖区人民医院2017年l月-2018年12月门诊中药房中药调剂内差资料作为研究对象,按照调剂内差的类型进行分类整理,分析比较增加新的中药调剂管理措施前后的调剂内差的差别,并进一步探讨影响中药调剂内差的相关因素,促进中药调剂措施管理办法的实施.结果:对24份中药调剂内差资料数据整理显示,2018年(增加新的中药调剂措施管理)调剂处方出现的内差如处方辨认不清或看错药名而错配平均值1.47明显低于2017年(按照日常管理制度实施调剂管理)4.71的平均值(P<0.05);内差如剂数数量不符的错误2018年(增加新的中药调剂措施管理)平均值0.99明显低于2017年(按照日常管理制度实施调剂管理)3.58的平均值(P<0.01);内差如调配草药时出现少配、多配、漏配、串斗、串碗的错误2018年(增加新的中药调剂措施管理)平均值1.83明显低于2017年(按照日常管理制度实施调剂管理)11.67的平均值(P<0.01).结论:中药调剂措施管理的实施使调剂处方出现的内差数量明显减少,有效控制了门诊中药房的调剂内差,减少了发放到患者手里的药品差错.

Objective:To study the factors affecting the internal difference of traditional Chinese medicine,to take control of the difference in the Outpatient pharmacy,and to strengthen and improve the managerial implementation of the Chinese medicine dispensing measures.Methods:The internal data were selected as the research subjects which belonged to the Outpatient pharmacy of Shenzhen Luohu District People's Hospital in the past two years(January 2017-December 2018),and according to the type of the difference in the adjustment,the analysis increased.They were sorted according to the type of difference in the adjustment.The difference in the adjustment of the new Chinese medicine after the management measures should be analyzed,and the relevant factors affecting the internal difference of the Chinese medicine should be explored,and the implementation of the management measures for the Chinese medicine adjustment should be promoted.Results:The internal data of 24 Chinese medicine preparations showed that the internal difference of the prescriptions in 2018(adding new Chinese medicine adjustment measures management)such as unclear or mismatched,whose average of 1.47 was lower than the average of 4.71(P<0.05)in 2017(according to the daily management system,the adjustment management);the internal difference of the prescriptions in 2018(adding new Chinese medicine adjustment measures management)such as that the number of doses does not match the error,whose average of 0.99 was lower than the average of 3.58(P<0.01)in 2017(according to the daily management system,the adjustment management);the internal difference of the prescriptions in 2018(adding new Chinese medicine adjustment measures management)such as errors of less,more,missing,skipping,and bowling,whose average of 1.83 was lower than the average of 11.67(P<0.01)in 2017(according to the daily management system,the adjustment management).Conclusion:The implementation of the traditional Chinese medicine adjustment measures has significantly reduced the number of internal differences in the prescriptions,effectively controlled the difference in the adjustment of the outpatient pharmacies,and reduced the errors in the medicines distributed to patients.
