摘要 目的:研究彩色多普勒超声闪烁伪影对输尿管结石成分分析.方法:回顾性分析2018年5月-2019年5月在本院诊断为输尿管结石的142例患者的临床资料.患者均行彩色超声闪烁伪影检查,对结石声影情况行相关记录,根据患者闪烁伪影及声影预测结石的成分,再对手术或自然排石后的结石标本以红外光谱自动分析法进行成分分析.比较预测结果与成分分析结果是否一致,探究不同结石成分与患者性别、年龄之间的关系.结果:142例患者中,含草酸钙为76例、含尿酸22例、含磷酸钙3例、混合结石41例.彩色多普勒超声闪烁伪影敏感度为61.1%,特异度为76.9%,阳性预测值为77.6%,阴性预测值为54.7%,AUC为0.679.不同年龄的结石成分构成比中除磷酸钙外,草酸钙、尿酸、混合结石的男性检出率均高于女性.不同年龄的结石成分构成比中,>60岁者草酸钙成分较多,46~60岁者混合结石成分较多(P<0.05).结论:应用彩色多普勒闪烁伪影分析输尿管结石成分基本可行. Objective:To study the component analysis of ureteral calculi by color Doppler ultrasound scintillation artifact.Method:The clinical data of 142 cases of ureteral calculi diagnosed in our hospital from May 2018 to May 2019 were analyzed retrospectively.All patients were examined by color ultrasound scintillation artifact,and the sound and shadow of stones were recorded.According to the scintillation artifact and sound shadow,the components of calculi can be predicted.Then the components of the stone specimens after surgery or natural lithotomy were analyzed by infrared spectrum automatic analysis.The correlation between the predicted results and the component analysis results was compared,the relationship between different stone components and the patient’s gender and age were explored.Result:Among the 142 patients,there were 76 cases containing calcium oxalate,22 cases containing uric acid,3 cases containing calcium phosphate and 41 cases containing mixed calculi.The sensitivity,specificity,positive predictive value,negative predictive value,and AUC were 61.1%,76.9%,77.6%,54.7%,and 0.679 respectively.Except calcium phosphate,the detection rate of calcium oxalate,uric acid and mixed calculi in males at different ages were higher than those in females.Among the stone composition ratios of different ages,the patient’s of more than 60 years old had more calcium oxalate components,and those aged 46 to 60 years old had more mixed stone components(P<0.05).Conclusion:It is feasible to analyze the components of ureteral calculi by color Doppler scintillation artifacts.
