摘要
目的:探讨Allgower-Donati缝合结合围手术期中药治疗在预防跟骨骨折术后切口并发症中的效果。方法:选取本院2014年1月-2019年10月收治的闭合性跟骨骨折患者60例,按随机数字表法分为对照组与观察组,各30例。两组均采用外侧"L"形切口切开复位内固定手术治疗。对照组采用传统缝合方法,围术期采用常规消肿及抗炎治疗。观察组采用Allgower-Donati缝合结合围术期中药治疗。比较两组围手术期相关指标、术后情况及治疗效果。结果:两组手术时间、手术切口缝合时间及引流管留置时间比较,差异均无统计学意义(P>0.05)。观察组患肢足部肿胀时间及切口渗出时间均短于对照组,且切口周围皮缘坏死及感染发生率均低于对照组,差异均有统计学意义(P<0.05)。观察组治疗优良率高于对照组(P<0.05)。结论:Allgower-Donati缝合结合围手术期中药治疗可有效地缩短跟骨骨折患者术后患肢足部肿胀及切口渗出时间,并减少术后手术切口皮缘坏死及感染的发生,值得临床广泛推广应用。
Objective:To explore the effect of Allgower-Donati suture combined with perioperative traditional Chinese medicine treatment on the prevention of postoperative incision complications of calcaneal fracture.Method:A total of 60 patients with closed calcaneal fractures from January 2014 to October 2019 in our hospital were selected.They were divided into control group and observation group with a random number table method,30 cases in each group.Both groups were treated with open reduction and internal fixation through lateral“L”incision.The control group was treated with traditional suture method and perioperative conventional detumescence and anti-inflammatory therapy.The observation group was treated with Allgower-Donati suture combined with perioperative traditional Chinese medicine treatment.Perioperative indicators,postoperative conditions and therapeutic effects were compared between the two groups.Result:There were no significant differences in operation time,incision suture time and drainage tube indwelling time between the two groups(P>0.05).The foot of the affected limb swelling and incision exudation time in the observation group were lower than those in the control group,and the incidence of incision skin edge necrosis and infection were lower than those in the control group,the differences were statistically significant(P<0.05).The excellent and good rate of treatment in the observation group was higher than that in the control group(P<0.05).Conclusion:Allgower-Donati suture combined with perioperative traditional Chinese medicine treatment can effectively shorten postoperative foot of the affected limb swelling and incision exudation time of patients with calcaneal fracture,and reduce postoperative incision skin edge necrosis and infection,which is worthy of clinical application.
