乳腺癌配偶照顾者连带病耻感的影响因素分析

Effect Factors of Joint Stigma in Breast Cancer Spouse Caregivers

目的:探讨连带病耻感对乳腺癌配偶照顾者负担的影响.方法:选取2017年8月-2019年3月于本院治疗的78例乳腺癌患者及其配偶照顾者,门诊随访时由配偶照顾者填写人口学调查问卷及连带病耻感量表,分析乳腺癌配偶照顾者连带病耻感的影响因素.结果:78例患者中按时门诊随访并填写问卷73份,不合格问卷3份,有效问卷为70份.不同年龄、病程和生活自理能力患者的配偶照顾者连带病耻感评分比较,差异均有统计学意义(P<0.05).不同年龄和每日照顾时间的配偶照顾者连带病耻感评分比较,差异均有统计学意义(P<0.05),其中可独立生活患者的配偶照顾者连带病耻感评分均低于部分与完全需要照顾患者(P<0.05).患者年龄≥60岁和照顾者年龄≥60岁是配偶照顾者连带病耻感的保护因素(P<0.05),而患者生活需要部分照顾或完全照顾、病程时间增长以及每日照顾时间增长均为配偶照顾者连带病耻感的独立危险因素(P<0.05).结论:中青年乳腺癌患者配偶照顾者连带病耻感程度相比老年患者更加严重,患者自理能力、病程及配偶照顾者照顾时间是影响乳腺癌配偶照顾者连带病耻感水平的主要危险因素.

Objective:To investigate the effect factors of joint stigma in breast cancer spouse caregivers.Method:A total of 78 breast cancer patients treated in our hospital and their spouse caregivers from August 2017 to March 2019 were selected.During the outpatient follow-up,the spouse caregivers filled in the demographic questionnaire and associated disease shame scale,the influencing factors of joint stigma in breast cancer spouse caregivers were analyzed.Result:Among 78 patients,73 copies were followed up and filled in questionnaires,3 copies were unqualified and 70 copies were valid.There were statistically significant differences in the joint stigma scores of spouses caregivers of patients with different age,disease course,and self-care ability(P<0.05).There were statistically significant differences in the in the joint stigma scores of spouses caregivers of spouses caregivers with different ages and daily care time(P<0.05),among them,the joint stigma scores of spouses caregivers of patients with independent living were lower than those of patients with partial or complete need for care(P<0.05).Patients’age≥60 years and caregivers’age≥60 years were protective factors for spouse caregivers’joint stigma(P<0.05),while patient with partial or complete need for care in life,increased duration of disease and increased daily care time were independent risk factors for spouse caregivers’joint stigma(P<0.05).Conclusion:The degree of joint stigma in spouse caregivers of young and middle-aged breast cancer patients is more serious than that of elderly patients.The self-care ability,disease course and caring time of spouse caregivers are the main risk factors affecting the level of joint stigma in breast cancer spouse caregivers.
