摘要 目的调查上海市安亭镇户籍女性人乳头瘤病毒(human papilloma virus,HPV)感染情况,为未来本地区宫颈癌预防和疫苗研制工作提供参考依据。方法采用分层整群抽样的方法进行抽样,对所有抽样人群的宫颈脱落细胞进行采样,用基因测序法检测HPV基因亚型。结果本次共采集到4 781例有效样本,其中HPV阳性484例,HPV感染率为10.12%,其中高危型HPV感染465例,感染率为9.73%,低危型HPV感染19例,感染率为0.39%。单一感染411例,以单一高危感染为主,占95.62%;多重感染73例,以两重高危感染为主,占72.60%。各年龄组HPV感染率:21~30岁组为9.84%,31~40岁组为9.77%,41~50岁组为10.14%,51~60岁组为9.48%,61~65岁组为17.19%,以61~65岁年龄组的感染率最高。各年龄组间感染率差异有统计学意义(χ^2=39.91,P<0.05)。16种HPV基因亚型,按感染率高低前四位依次为HPV52型、58型、16型和68型,感染率依次为2.64%、1.55%、1.34%和1.11%。结论上海市安亭镇户籍女性HPV感染率处于较低水平,但未来仍需加强对中老年女性HPV和宫颈癌的健康宣教工作,继续推进HPV疫苗研制工作。 OBJECTIVE To investigate the infection status of human papilloma virus(HPV)in Shanghai Anting Town,and to provide ref⁃erence for cervical cancer prevention and vaccine development.METHODS The cervical exfoliated cells were collected in population sampled by stratified cluster sampling,and the HPV genotype was detected by gene sequencing.RESULTS 4781 valid samples were collected,and 484 cases were HPV infected,the infection rate of HPV were 10.12%,among them 465 cases were high risk HPV(HR-HPV)infection,the infection rate of HR-HPV were 9.73%,19 cases were low risk HPV(LR-HPV)infection,the infection rate of LR-HPV were 0.39%.There were 411 cases with single HPV type infection,73 cases with multiple HPV subtypes infection.HPV infection rate in different age groups were different,and 61~65 years women had the highest HPV infection rate.The difference among different age groups was statistically significant(χ^2=39.91,P<0.05).The top four HPV genotypes were HPV52,58,16 and 68,and the infection rates respectively were 2.64%,1.55%,1.34%and 1.11%.CONCLUSION The HPV infection rate of household regis⁃tered women in Anting,Shanghai was at a low level.But in the future,it was still necessary to strengthen the health education of HPV and cervical cancer for middle-aged and old women,and continue to promote the development of HPV vaccine.
