中西医结合治疗盆腔炎(湿热瘀结型)的临床疗效观察

Clinical Observation on the Treatment of Pelvic Inflammation(Damp-heat and Blood Stasis)with the Combination of Tradi⁃tional Chinese and Western Medicine

目的观察中西医结合治疗盆腔炎(湿热瘀结型)的临床疗效。方法收集2018年3月-2019年4月在上海市青浦区中医医院住院的慢性盆腔炎患者80例,随机分为对照组40例和治疗组40例,对照组以西医补液抗炎(头孢替安+替硝唑+维生素C)连续治疗2周,治疗组在西医补液抗炎治疗的基础上,加用中药饮片和中医特色操作。中药汤方以清热调血汤为基础方,随症加减治疗,加用穴位敷贴(关元、气海和三阴交),治疗1周后再加用红外线治疗。疗程结束后,观察患者下腹及腰骶胀痛,带下的量、色、质及次症的总积分变化情况;观察患者体征变化、中医证候等疗效指标,比较两组患者的临床疗效。结果治疗组总有效率为92.5%,对照组总有效率为75.0%,两组有效率比较有显著差异(P<0.05);中医症候评分治疗组总有效率为85.0%,对照组总有效率为65.0%,两组有效率比较有显著差异(P<0.05),随着治疗时间的增加中医症候评分下降明显,差异有统计学意义(P<0.05)。结论清热调血汤联合穴位敷贴、红外线治疗慢性盆腔炎疗效确切,在症状、体征改善方面有明显作用,具有明显的疗效和良好的安全性。

OBJECTIVE To observe the clinical effect of the combination of traditional Chinese and Western Medicine on pelvic inflam⁃mation(Damp-heat and Blood Stasis).METHODS From March 2018 to April 2019,80 patients with chronic pelvic inflammatory dis⁃ease were randomly divided into control group(40 cases)and experimental group(40 cases).In the control group,40 cases were treated with western medicine for 2 weeks(cefotiam+tinidazole+vitamin C).In the experimental group,on the basis of Western medicine for anti-inflammatory treatment,Chinese herbal pieces and Chinese characteristic operation were added.Chinese herbal decoction was based on Qingretiaoxue,which was treated with symptoms,acupoint application(Guanyuan,Qihai,Sanyinjiao),and infrared treatment was added one week later.After the course of treatment,observe the patient’s hypogastralgia and lumbosacral pain,the change of the quantity,color and quality of leucorrhea,the total score of secondary symptoms,as well as the change of the patient’s physical signs and TCM syndromes,and compare the clinical efficacy results of these two groups.RESULTS The total effective rate was 92.5%in the experimental group and 75%in the control group,with significant difference between the two groups(P<0.05),in TCM syn⁃drome score,the total effective rate was 85%in the treatment group and 65%in the control group.There were significant differences between the two groups(P<0.05),with the increase of treatment time,TCM syndrome score decreased significantly(P<0.05).CON⁃CLUSION Qingre tiaoxue combined with acupoint application and infrared treatment was effective in the treatment of chronic pelvic in⁃flammatory disease,which had obvious effect on the improvement of symptoms and signs,and had obvious curative effect and good safety.
