辨证中药熏蒸疗法结合康复治疗对脑梗死后早期肩-手综合征的疗效

The Curative Effect of Traditional Chinese Medicine Fumigation Therapy Combinated with Rehabilitation on Early Shoulder-Hnd Syndrome after Cerebral Infarction

目的:评估辨证中药熏蒸疗法对脑梗死后早期肩-手综合征(shoulder-hand syndrome,SHS)的疗效。方法:脑梗死后早期SHS患者126例随机分入治疗组和对照组,每组各63例,对照组进行基础治疗和康复治疗,治疗组在此基础上进行辨证中药熏蒸疗法,采用患肢Fugl-Meyer评分评估运动功能,视觉模拟评分(visual analog scale,VAS)、painDETECT问卷积分评估疼痛程度,肿胀前臂周径差积分评估患肢肿胀程度,并评估辨证中药熏蒸疗法结合康复治疗对脑梗死后早期SHS的总疗效。结果:经过4周治疗后,中药熏蒸疗法结合康复治疗对脑梗死后早期SHS总有效率达86.88%。与对照组相比,治疗组患者患肢Fugl-Meyer评分增加、VAS、painDETECT问卷积分、肿胀前臂周径差积分均下降,差异具有统计学意义(P<0.05或P<0.01)。结论:辨证中药熏蒸疗法结合康复治疗对脑梗死后早期SHS有较好的疗效,并且副作用较小。

Objective:To evaluate the therapeutic effect of fumigation therapy of TCM based on syndrome differentiation on shoulder hand syndrome(SHS)in early stage after cerebral infarction.Methods:126 patients with SHS in the early stage after cerebral infarction were randomly divided into the treatment group and the control group,63 in each group and the control group on the basis of treatment and rehabilitation treatment,treatment group on the basis of traditional Chinese medicine fumigation therapy,syndrome differentiation with limb Fugl-Meyer score evaluation function,visual analogue scale(visual analog scale,VAS),integral painDETECT questionnaire evaluation degree of pain,swelling of the forearm radius zhou integral evaluation of limb swelling degree,and to assess syndrome after Chinese medicine fumigation therapy combined with rehabilitation therapy for cerebral infarction in the early general curative effect of SHS.Results:The total effective rate of rehabilitation combinated with dialectic fumigation and steaming therapy of traditional Chinese medicine on early SHS after cerebral infarction is 86.88%after 4 weeks treatment.Compared to patients in control group,the score of Fugl-Meyer of affected limb was increased,the score of VAS、painDETECT and integral value of difference of forearm circumference were downregulated after treatment,the difference was statistically significant(P<0.05 or P<0.01).Conclusion:TCM fumigation combined with rehabilitation therapy has a better effect on early SHS after cerebral infarction,and has less side effects.
