腮腺基底细胞腺瘤的CT诊断价值

CT diagnostic value of basal cell adenoma in parotid gland

目的:探讨腮腺基底细胞腺瘤(BCA)患者的临床及CT表现特点。方法:选取我院经手术病理证实的腮腺BCA患者16例为研究对象,均行CT增强扫描检查,结合病理结果分析其影像学特点。结果:发病年龄56~72岁,平均63.8±8.35岁,其中男5例,女11例,男女发病比例为1:2.2。16例患者共18个病变:单侧单发14个,双侧单发4个;15个位于腮腺浅叶,2个位于深叶,1个跨深浅叶生长;9个病变紧邻包膜下,位置表浅;16个强化不均匀、出现囊变,2个强化均匀;“快进快出”模式6例(33.3%),“快进慢出”模式12个(66.6%);中央裂隙影6个,薄壁环形强化且有壁结节9个。结论:腮腺BCA好发于中老年女性,CT表现为腮腺浅叶的类圆形结节,边缘清楚,极易发生囊变,囊变部位多位于中央部,部分病灶可见中央裂隙样瘢痕及钙化,增强CT扫描多呈“快进慢出”的模式,且多伴有薄壁环形强化且有壁结节,首先考虑腮腺基底细胞腺瘤诊断可能。

Objective To investigate the clinical and CT features of basal cell adenoma(BCA)of parotid gland.Methods 60 patients with parotid gland BCA confirmed by surgery and pathology in our hospital were selected as the study objects.All of them underwent enhanced CT scan,and their imaging characteristics were analyzed in combination with the pathological Results.Results The age of onset ranged from 56 to 72 years old,with an average age of 63.8±8.35 years old.There were 18 cases of lesions in 16 patients:14 cases were unilateral and 4 cases were bilateral;15 cases were located in superficial lobe of parotid gland,2 cases were located in deep lobe,and 1 case was grown across deep and shallow lobe;the lesions in 9 cases were adjacent to the subcapsule and superficial;the enhancement was uneven and cystic in 16 cases,and the enhancement was uniform in 2 cases;6 cases were“fast in and out”mode(33.3%)and 12 cases were“fast in and slow out”mode(66.6%);6 cases were central fissure shadow and 9 cases were thin-wall annular enhancement with wall nodules.Conclusion Parotid gland BCA mostly occurs in middle-aged and old women.CT findings showed that the superficial lobe of the parotid gland had circular nodules with clear margins,which were prone to cystic changes.The cystic changes were mostly located in the central part,and some lesions showed central fissure scar and calcification.The enhanced CT scan was mostly in the mode of"fast in and slow out",and often accompanied with thin-walled ring enhancement and wall nodules.The possibility of diagnosis of basal cell adenoma of parotid gland was first considered.
