TIP30、Vav3的表达与胃癌临床病理参数及预后的相关性

Correlation of TIP30 and Vav3 with clinicopathological parameters and prognosis of gastric cancer

目的探讨TIP30、Vav3的表达水平与胃癌临床病理参数及预后的相关性。方法收集2015年1月至2016年1月收治的120例行胃癌切除术病人的胃癌组织和癌旁组织,采用免疫组化染色测定TIP30、Vav3表达情况。随访3年,记录病人死亡情况,并分析胃癌组织TIP30、Vav3的表达与胃癌临床病理参数及预后的相关性。结果所有病人均获得随访,中位随访时间为35个月(2~46个月),死亡48例,存活52例。癌组织的TIP30表达阳性率低于癌旁组织(P<0.01),Vav3表达阳性率高于癌旁组织(P<0.01)。TIP30阳性表达组和Vav3阳性表达组病人的肿瘤直径、分化程度、TNM分期、淋巴结转移、血管浸润与阴性表达组比较,差异均有统计学意义(P<0.05或P<0.01)。单因素分析显示,胃癌病人预后与肿瘤直径、TNM分期、淋巴结转移、血管浸润、TIP30、Vav3的表达有关(P<0.05或P<0.01)。多因素COX回归分析显示,TIP30及Vav3的表达、TNM分期是影响胃癌病人预后的独立危险因素。TIP30、Vav3及联合应用预测胃癌预后的AUC分别为0.834、0.712、0.893。结论胃癌组织中TIP30、Vav3表达异常,与肿瘤直径、分化程度、TNM分期、淋巴结转移、血管浸润等病理参数存在相关性,是预测胃癌不良预后的有效指标。

Objective To explore the correlation of the expression of TIP30 and Vav3 with the clinicopathological parameters and prognosis of gastric cancer.Methods The gastric cancer tissues and paracancerous tissues of 120 patients undergoing gastric cancer resection admitted from January 2015 to January 2016 were collected,and the expression of TIP30 and Vav3 was determined by immunohistochemical staining.All the patients were followed up for 3 years.The prognosis was recorded,and the correlation of the expression of TIP30 and Vav3 in gastric cancer tissues with the clinicopathological parameters and prognosis were analyzed.Results All the patients were followed up,with a median follow-up time of 35 months(ranging from 2 to 46 months),with 48 deaths and 72 survivors.The positive rate of TIP30 in the cancer tissues was lower than that in the adjacent tissues(P<0.01),and the positive rate of Vav3 was higher(P<0.01).The expression of TIP30 and Vav3 was significantly different in the patients with different tumor diameter,differentiation,TNM stage,lymph node metastasis and vascular invasion(P<0.05 or P<0.01).Single factor analysis showed that tumor diameter,TNM stage,lymph node metastasis,vascular invasion,the expression of TIP30 and Vav3 were the influencing factors of prognosis(P<0.05 or P<0.01).COX regression analysis indicated that the expression of TIP30,Vav3 and TNM stage were independent risk factors for prognosis.The area under the receiver operating characteristic curve for predicting the prognosis of TIP30,Vav3 and combined application was 0.834,0.712 and 0.893,respectively.Conclusions Abnormal expression of TIP30 and Vav3 in gastric cancer tissues is correlated with the pathological parameters such as tumor diameter,differentiation degree,TNM stage,lymph node metastasis and vascular invasion,and which are effective indexes for predicting poor prognosis.
