超声弹性成像在甲状腺良恶性肿瘤鉴别诊断中的应用效果 被引量:1

摘要 目的观察超声弹性成像在甲状腺良、恶性肿瘤鉴别诊断中的应用价值。方法选择我院甲状腺肿瘤患者实施此次研究,例数为1500例,时间为2017年11月至2018年11月,所有患者均实施超声弹性成像检查及常规超声检查,将病理组织检查作为金标准,观察两种检查方法的诊断准确性。结果本组甲状腺肿瘤手术患者1500例,确诊良性肿瘤1320例,恶性肿瘤180例。超声弹性成像检查发现良性肿瘤1330例,恶性肿瘤170例;常规超声检查发现良性肿瘤1240例,恶性肿瘤260例。常规超声诊断:灵敏度为91.11%,特异度为94.24%,准确度为92.53%;超声弹性成像诊断:灵敏度为90.55%,特异度为99.46%,准确度为98.40%。两种检查方法诊断灵敏度对比无显著差异,超声弹性成像准确度及特异度高于常规超声检查,有统计学意义(P<0.05)。结论在甲状腺良恶性肿瘤诊断中应用超声弹性成像可提升诊断准确率,临床应用及推广价值极高。 Objective To observe the value of ultrasound elastography in the differential diagnosis of benign and malignant thyroid tumors.Methods The study was conducted in patients with thyroid tumors in our hospital.The number of cases was 1500.The time was from January 2017 to January 2018.All patients underwent ultrasound elastography and routine ultrasonography.The pathological examination was performed as gold.Standard,observe the diagnostic accuracy of the two methods of examination.Results This group of 1500 patients with thyroid tumor surgery,1320 confirmed benign tumors,180 malignant tumors.Ultrasound elastography revealed 1330 benign tumors and 170 malignant tumors;routine ultrasound examination revealed 1240 benign tumors and 260 malignant tumors.Conventional ultrasound diagnosis:sensitivity is 91.11%,specificity is 94.24%,accuracy is 92.53%;ultrasound elastography diagnosis:sensitivity is 90.55%,specificity is 99.46%,accuracy is 98.40%.There was no significant difference in the diagnostic sensitivity between the two examination methods.The accuracy and specificity of ultrasound elastography were higher than those of conventional ultrasound(P<0.05).Conclusion The application of ultrasound elastography in the diagnosis of benign and malignant thyroid tumors can improve the diagnostic accuracy,and the clinical application and promotion value is extremely high.
