新生期缺血缺氧大鼠脑梗死后癫痫PET/CT癫痫源定位诊断探讨

Experimental study on the characteristics of cerebral glucose metabolism in rats with per-inatal hypoxia-induced epilepsy by 18F-FDG micro-PET/CT

摘要 目的:探讨围产期缺血缺氧脑梗死后癫痫发作大鼠模型微-PET/CT的18F-FDG特点,为其脑梗死后癫痫源定位提供实验依据。方法:选取96只健康15d龄新生期大鼠用于实验,随机分为正常组A组、缺血缺氧模型组B组、假手术组C组、缺氧组D组,每组24只。于出生后23d进行断奶喂养后,对每组24只大鼠分别进行行为学观察(8只)、微-PET/CT显像(8只)、脑电图监测(8只)等。采用SPSS 17.0统计软件进行整理和分析,计量资料采用均数士标准差(x^-±s),t检验。结果:行为学观察到A、B、C、D各组自发性癫痫发作大鼠Racine分级≥3级的只数分别为:0只、3只、1只和2只。癫痫发作持续时间为5~137s不等。B组鼠癫痫平均发作持续时间与A组比较差异有统计学意义(P<0.05),B组大鼠脑电图频发的痫性尖波、棘波,随着鼠龄增大,痫样波出现频率逐渐降低,B组大鼠PET/CT提示目标梗死灶SUV(标准化摄取值)与非梗死侧大脑半球所对应脑区SUV值比较,代谢不对称指数AI>15%,且梗死灶周围脑区脑电图检测捕捉到癫痫波。A、C组脑电图基本正常,D组为轻度异常脑电图。结论:围产期缺血缺氧脑梗死后癫痫发作大鼠l8F-FDG微-PET/CT通过反映其脑组织代谢情况,可用于癫痫源的定位。 Objective:To investigate the distribution characteristics of 18F-FDG micro-PET/CT of the rat models with perinatal ischemic hypoxia-induced epileptic seizures,so as to provide the exper-imental evidence for the study of the localization of epileptogenic focus in rats.Methods:The above mentioned treatment was applied to establish an ischemic stroke epilepsy models.96 healthy rats were divided into 4 groups:Group A(the normal controls),Group B(hypoxic ischemic models),Group C(sham-operated cases)and Group D(hypoxic cases),with 24 rats in each.The monitoring time ranged from 23 days after birth,3 rats were randomly selected from each group for micro-PET/CT imaging,and the analysis was made on the glucose uptake values of each part of brains.The rats in each period were tested by EEG in the target area.Results:The number of the rats with Racine scores≥3 in different groups was 0,3,1 and 2 respectively.The duration of seizures ranged from 5 to 137s.The frequency of seizures in the model group was the highest.In Group B,the EEG showed frequent epileptic spikes,but with the increase of age,the frequency of epileptic waves gradually de-creased.Paroxysmal tip-spur waves were found in Groups A,C,and D occasionally,and the frequen-cy of occurrence was significantly lower than that of Group B.Conclusion:Micro-PET/CT can be used to localize brain metabolism in rats with perinatal ischemic hypoxia induced epileptic seizures.
