集束化护理在心内科中心静脉导管维护中的应用

Application of cluster nursing in maintenance of central venous catheter in department of cardiology

目的探讨集束化护理在内科中心静脉导管(CVC)维护中的应用价值。方法选取2016年8月~2018年8月我院诊治的258例CVC患者作为研究对象,按患者就诊顺序将其分成观察组和对照组。2016年8月~2017年8月诊治的138例为对照组,2017年9月~2018年8月诊治的120例为观察组。对照组给予常规置管护理措施,观察组在对照组的基础上给予集束化护理,比较两组CVC导管相关血流感染率、非计划拔管率、置管和住院时间。结果观察组CVC相关性血流感染率低于对照组,差异有统计学意义(P<0.05);观察组非计划拔管率低于对照组,差异有统计学意义(P<0.05)。观察组置管和住院时间短于对照组,差异均有统计学意义(P<0.05)。结论集束化护理能有效减少CVC相关性血流感染率和非计划拔管率,且缩短患者置管时间和住院时间。

Objective To explore the application value of cluster nursing in the maintenance of central venous catheter(CVC)in the department of cardiology.Methods A total of 258 patients diagnosed and treated in our hospital from August 2016 to August 2018 were selected as subjects.The patients were divided into the observation group and the control group according to the order of diagnosis and treatment.Among them,138 patients diagnosed and treated from August 2016 to August 2017 served as the control group,and 120 patients from September 2017 to August 2018 served as the observation group.The control group was given routine catheterization nursing measures,and the observation group was given cluster nursing on the basis of the control group.The CVC catheter-related blood flow infection rate,unplanned extubation rate,catheterization and hospitalization days were observed and compared between the two groups.Results The infection rate of CVC related blood flow in the observation group was lower than that in the control group(P<0.05),and the rate of unplanned extubation in the observation group was lower than that in the control group,with statistical differences(P<0.05).In addition,the days of catheterization and hospitalization in the observation group were also shorter than those in the control group,with statistical differences(P<0.05).Conclusion Cluster nursing can effectively reduce the CVC catheter-related blood flow infection rate and unplanned extubation rate,and reduce the catheterization time and hospitalization days of patients.
