中医药介入的快速康复外科在肛周脓肿和肛瘘术后的应用研究

Application Research of Traditional Chinese Medicine Intervention in Enhanced Recovery after Surgery of Perianal Abscess and Anal Fistula

目的:观察肛周脓肿和肛瘘术后中医药介入在快速康复中的作用。方法:将79例肛周脓肿和肛瘘术后患者随机分为治疗组39例和对照组40例。对照组采用肛周脓肿、肛瘘常规临床路径方案;治疗组采用中医药介入的快速康复外科治疗方案。观察两组患者术后的疼痛评分,舒适度评分,创面愈合时间。结果:两组术后第1天活动状态疼痛分值比较,无统计学差异(P>0.05);术后第3天、第5天、第7天,有统计学差异(P<0.05)。两组术后第7天患者舒适状况比较,有统计学差异(P<0.05)。两组术后创面愈合时间比较,有统计学差异(P<0.05)。结论:中医药介入的快速康复外科治疗方案,能够降低肛周脓肿和肛瘘患者术后第3天、第5天、第7天的的疼痛,提高整体舒适度,加快伤口愈合。

Objective: To investigate the application of TCM intervention in rapid rehabilitation after perianal abscess and anal fistula.Methods: 79 patients with perianal abscess or anal fistula were randomly divided into two groups,a treatment group(39 cases) and a controlled group(40 cases).The controlled group adopted routine clinical pathway for perianal abscess and anal fistula,while the treatment group adopted a therapeutic regimen of enhanced recovery after surgery with traditional Chinese medicine.Pain score,comfort score and healing time were observed in two groups.Results: There was no significant difference in pain score of active state on the first day after operation between the two groups,but there was significant difference on the third,fifth and seventh day after operation.There was statistical difference in comfort score between the two groups on the 7 th day after operation.There was significant difference in healing time between the two groups.Conclusion: The enhanced recovery after surgery with TCM intervention can reduce the pain of patients on the 3 rd,5 th and 7 th day after operation,also can improve comfort and accelerate wound healing.
