针灸治疗对脑卒中后偏瘫颈肩痛患者的疼痛改善及预后的影响分析

Analysis on the Effect of Pain Improvement and Prognosis for Patients with Hemiplegia and Shoulder Pain after Stroke with Acupuncture Therapy

目的:观察针灸治疗对脑卒中后偏瘫颈肩痛患者的疼痛改善及预后的影响。方法:选择2014年6月~2016年6月,在我院进行治疗的脑卒中后偏瘫颈肩痛患者72例进行研究。按随机数表法随机分为对照组和观察组。对照组患者使用常规镇痛治疗,观察组在此基础上结合针灸治疗。治疗周期均1个月。观察两组治疗后的临床疗效,中医证候疗效。并对比两组患者治疗前、后的神经功能评分(NDS)及日常生活能力评分(BI)。结果:两组患者治疗前后颈肩背痛、颈项部肌肉压痛和紧张度评分比较,观察组各项数据均优于对照组,差异有统计学意义(P<0.05)。治疗后NDS、BI评分比较结果显示,NDS、BI总有效率分别为94.4%、97.2%,明显高于对照组(66.7%、58.3%),差异有统计学意义(P<0.05)。治疗后的NRS-101评分比较,观察组优于对照组,差异有统计学意义(P<0.05)。两组患者治疗后,观察组不良反应率8.33%,高于对照组33.33%。两组比较,差异有统计学意义(P<0.05)。结论:针灸治疗脑卒中后偏瘫颈肩痛患者,有显著疗效,能够明显改善患者的日常生活能力,提高患者生活质量。

Objective:To study on acupuncture treatment of hemiplegia and shoulder pain for patients with pain treatment effect and prognosis effect after stroke.Methods:76 patients chose from June 2014 to June 2016 who attacked with stroke with hemiplegia after neck pain in our hospital,were randomly divided into controlled group and observation group.The patients in the controlled group were treated with routine analgesics and exercise therapy.The observation group was treated with acupuncture at the acupoint of shoulder pain on the basis of this,and the treatment period was one month.The clinical curative effect of the observation group after treatment,the curative effect of TCM syndrome.The neurological function(NDS)and daily living ability score(BI)after treatment were compared between the two groups.Results:The two groups of patients before and after the treatment of neck and back pain,neck tenderness and muscle tension score compared with the data of the observation group were better than the controlled group,the difference was statistically significant(P<0.05).After treatment,the NDS and BI scores showed that the total effective rates of NDS and BI were 94.4%and 97.2%,respectively,which was much higher than in the controlled group(66.7%,58.3%,P<0.05).After the treatment,the NRS-101 score of the observation group was better than the controlled group,and the difference was statistically significant(P<0.05).After treatment in the two groups,the rate of adverse reaction in the observation group was 8.33%,which was higher than that of the controlled group(33.33%).The difference between the two groups was statistically significant(P<0.05).Conclusion:Acupuncture treatment of hemiplegia after stroke in patients with neck pain,has obvious curative effect,can significantly improve the ability of daily life of patients,improve the quality of life of patients.
