摘要
目的:探讨海南黎族人人类白细胞抗原-G(HLA-G)基因外显子8中14 bp多态性分布与重度子痫前期易感性的相关性。方法:选择2018年6月~2019年9月本院收治的100例重度子痫前期住院患者为实验组,其中黎族50例,汉族50例,另选择同期本院妇产科收治的100例正常妊娠孕妇为对照组,其中黎族50例,汉族50例。均采集静脉血检测HLA-G基因外显子8中14 bp多态性,分析HLA-G基因外显子8中14 bp多态性与重度子痫前期易感的相关性。结果:对照组黎族与实验组黎族HLA-G基因外显子8中14 bp基因分型和等位基因型频率相比,差异有统计学意义(P<0.05)。实验组黎族-14 bp/-14 bp分型、+14 bp/-14 bp分型、等位基因-14 bp分型患者SBP、DBP低于实验组汉族患者(P<0.05),+14 bp/-14 bp分型SBP、DBP高于实验组汉族患者(P<0.05)。二元Logistic回归分析+14 bp/-14 bp与海南地区黎族女性重度子痫前期的发病有关(P<0.05),-14 bp/-14 bp分型是海南地区黎族女性重度子痫前期的保护性因素(P<0.05)。结论:海南黎族HLA-G基因外显子8携带14 bp缺失多态性与重度子痫前易感和进展有关。
Objective:To explore the correlation between the distribution of 14bp polymorphism in exon 8 of human leukocyte antigen-G(HLA-G)gene in Hainan Li nationality and susceptibility to severe preeclampsia.Methods:A total of 100 patients with severe preeclampsia admitted to Hainan Provincial People's Hospital from June 2018 to September 2019 were selected as the experimental group,including 50 patients from Li nationality and 50 patients from Han nationality.In addition,100 normal pregnant women admitted to the Department of Gynaecology and Obstetrics of the hospital during the same period were selected as the control group,including 50 women from Li nationality and 50 women from Han nationality.Venous blood was collected to detect the 14bp polymorphism in HLA-G gene exon 8,and the correlation between the 14bp polymorphism in HLA-G gene exon 8 and susceptibility to severe preeclampsia was analyzed.Results:There was a statistically significant difference in the 14bp genotyping and allele frequency in HLA-G exon 8 of Li nationality between the control group and the experimental group(P<0.05).In the experimental group,systolic blood pressure(SBP)and diastolic blood pressure(DBP)of the patients with-14bp/-14bp genotype,+14bp/+14bp genotype and allele-14bp genotype from Li nationality were lower than those from Han nationality(P<0.05),but SBP and DBP of the patients with+14bp/-14bp genotype from Li nationality were higher than those from Han nationality(P<0.05).Binary logistic regression analysis showed there was a significant association between+14bp/-14bp genotype and the incidence of severe preeclampsia in women from Li nationality in Hainan region(P<0.05).The-14bp/-14bp genotype was a protective factor for severe preeclampsia in women from Li nationality in Hainan region(P<0.05).Conclusions:The HLA-G gene exon 8 carrying a 14bp deletion polymorphism in the Hainan Li nationality is associated with preeclampsia susceptibility and progression.
