股骨重建髓内钉与加压钢板治疗股骨干骨折的疗效比较

Comparison of the curative effect of femoral reconstruction intramedullary nail and compression plate in the treatment of femoral shaft fracture

目的比较股骨重建髓内钉与加压钢板治疗股骨干骨折的疗效。方法将60例股骨干骨折患者随机分为两组,对照组采用加压钢板内固定治疗(30例),观察组采用股骨重建髓内钉治疗(30例)。记录两组术中出血量、手术时间、住院时间、骨折愈合时间等,比较两组术后膝关节功能。结果患者均获得12个月随访。术中出血量、住院时间、骨折愈合时间、去拐下地负重行走时间观察组均优于对照组(P<0.05);两组手术时间比较差异无统计学意义(P>0.05);术后12个月,HSS膝关节功能优良率观察组明显优于对照组(P<0.05)。结论与加压钢板内固定相比,股骨重建髓内钉治疗股骨干骨折效果更理想,具有术中出血量少、易愈合、康复及功能恢复快、固定牢固等优点。

Objective To compare the curative effect of femoral reconstruction intramedullary nail and compression plate in the treatment of femoral shaft fracture.Methods Sixty patients with femoral shaft fractures were randomly divided into two groups.The control group was treated with compression plate internal fixation(30 cases),and the observation group was treated with femoral reconstruction intramedullary nail fixation(30 cases).The intraoperative bleeding volume,operation time,hospitalization time and fracture healing time were recorded,and which were used to compare the knee joint function between the two groups.Results All patients were followed up for 12 months.The intraoperative bleeding volume,length of hospital stay,fracture healing time and walking time under weight in the observation group were better than those in the control group(P<0.05);there was no significant difference in operation time between the two groups(P>0.05);at 12 months after operation,the excellent and good rate of HSS knee joint function in the observation group was significantly better than that in the control group(P<0.05).Conclusions Compared with compression plate internal fixation,femoral reconstruction intramedullary nail fixation is more effective in the treatment of femoral shaft fracture.It has the advantages of less bleeding,easy healing,quick function recovery and firm fixation.
