分期治疗开放跟骨骨折

Staged treatment of open calcaneal fractures

目的探讨分期治疗开放跟骨骨折的效果。方法对25例合并内侧伤口的开放跟骨骨折患者行一期急诊清创,待内侧伤口稳定后,二期行切开复位内固定。末次随访时采用AOFAS踝-后足功能评分评定疗效。结果25例患者均获得15个月随访。23例外侧手术切口无感染、坏死发生;2例外侧伤口发生浅表感染,经换药后延迟愈合。结论开放跟骨骨折一期彻底清创、处理软组织,二期行切开复位内固定术,分期治疗效果满意。

Objective To investigate clinical outcomes of staged treatment of open calcaneal fracture.Methods The 25 patients of open calcaneal fractures with medial wounds were treated in one staged emergent debridement,when the medial wound situation was stable,open reduction and internal fixation were performed in the second stage.The AOFAS ankle-hindfoot score for ankle effect was performed at the final follow-up.Results All 25 patients were followed up for 15 months.The 23 cases wounds were healed without infection and necrosis,2 cases wounds were healed after change of dressing for superficial infection.Conclusions The open calcaneal fractures is treated by stage,and the debridement and soft tissue treatment is in one stage,open reduction and internal fixation is in the second stage,staged treatment are satisfactory.
