硬核白内障手术方法的临床观察

Clinical Observation of Hard Cataract Surgery

目的分析硬核白内障手术方法,并比较不同手术方法的临床效果。方法选择我院于2015年1月至2017年12月收治的78例硬核白内障患者作为本次研究的对象,并将其分为观察组与对照组,每组39例。观察组选择小切口白内障手法碎核伴折叠式人工晶体植入术,对照组选择超声乳化伴折叠式人工晶体植入术,在手术结束后比较两组患者的角膜散光度数与角膜内皮变化,结合并发症发生情况判断不同手术的临床效果。结果研究数据结果表明,观察组在角膜散光度数与角膜内皮减少量方面均要优于对照组,而在并发症发生情况上,观察组出现1例后囊膜破裂与1例虹膜损伤,而对照组出现了2例后囊膜破裂与3例虹膜损伤,并发症发生率上观察组也以5.1%优于对照组的12.8%。两组患者各项数据对比差异明显,具有统计学意义(P<0.05)。结论对于硬核白内障患者来说,小切口白内障手法碎核可以更好地控制患者的术源性散光,角膜内皮减少率及水肿发生率也更低,患者术后视力恢复更快,在并发症发生情况上也要更加稳定,因此可以作为今后的主要治疗方案。

Objective To analyze the surgical methods of hard nucleus cataract with adhesive pupil and compare the clinical effects of different surgical methods.Methods 78 cases of hard core cataract patients with adhesive small pupil admitted and treated in our hospital from January 2015 to December 2017 were selected as the objects of this study,and were divided into observation group and control group,39 cases in each group.Small incision extracapsular cataract extraction was selected in the control group,and phacoemulsification was selected in the observation group.After the operation,the changes of corneal astigmatism and corneal endothelium in the two groups were compared,and the clinical effects of different operations were judged by combining the incidence of complications.Results The data of research results show that the observation group with corneal endothelial reduction in corneal astigmatism degree were better than that in control group,and on the complication occurrence,observed group of posterior capsular rupture in 1 case and 1 case of iris damage,and the control group in the posterior capsular rupture in 2 cases and 3 cases of iris damage,what complications in observation group is superior to that in the control group with 5.1%12.8%.There were significant differences in the data between the two groups(P<0.05).Conclusion For patients with hard nucleus cataract,phacoemulsification can better control the intraoperative astigmatism of patients,better promote the visual recovery of patients,and be more stable in the occurrence of complications,so it can be used as the main treatment program in the future.
