术前肿瘤标志物检测对肝内胆管细胞癌淋巴结转移的预测价值

Predictive Value of Preoperative Tumor Marker Detection for Lymph Node Metastasis of Intrahepatic Cholangiocarcinoma

目的分析术前肿瘤标志物检测对肝内胆管细胞癌淋巴结转移的预测作用。方法选择我院于2017年1月至2018年11月收治的93例肝内胆管细胞癌患者为研究对象。依据术后病理检查结果,将其分成常规组(术后病理检查证实未发生转移,61例)、观察组(术后病理检查证实伴淋巴结转移,32例)。给予两组术前肿瘤标志物检测及术后病理检查。肿瘤标志物检测充分按照试剂说明书及全自动化学发光仪的检测要求完成,通过良好的质量控制保障肝内胆管细胞癌患者肿瘤标志物检测结果的准确性。结果观察组CA19-9、CA125、CA211、CA242水平均高于常规组(P<0.05)。CA19-9、CA125、CA211、CA242检测肝内胆管细胞癌淋巴结转移的符合率均低于术后病理检查(P<0.05)。结论淋巴结转移的发生会对肝内胆管细胞癌患者的肿瘤标志物水平产生影响,术前肿瘤标志物检测对肝内胆管细胞癌淋巴结转移具有一定的预测价值。

Objective To analyze the predictive effect of preoperative tumor markers on lymph node metastasis in intrahepatic cholangiocarcinoma.Methods Ninety-three patients with intrahepatic cholangiocarcinoma admitted to our hospital from January to February 2018 were enrolled.According to the results of postoperativ e pathological examination,it was divided into the conventional group(postoperative pathological examination confi rmed no metastasis,61 cases),observation group(postoperative pathological examination confi rmed lymph node metastasis,32 cases).Two groups of preoperative tumor markers and postoperative pathological examination were given.Tumor marker detection is completed in accordance with the reagent instructions and the detection requirements of the fully automatic chemiluminometer.The accuracy of the tumor marker detection results in patients with intrahepatic cholangiocarcinoma is guaranteed by good quality control.Results The levels of CA19-9,CA125,CA211 and CA242 in the observation group were higher than those in the conventional group(P<0.05).The coincidence rates of CA19-9,CA125,CA211,CA242 in detecting lymph node metastasis of intrahepatic cholangiocarcinoma were lower than postoperative pathological examination(P<0.05).Conclusion The occurrence of lymph node metastasis affects the level of tumor markers in patients with intrahepatic cholangiocarcinoma.Preoperative tumor marker detection has certain predictive value for lymph node metastasis of intrahepatic cholangiocarcinoma.
