IKAP模式护理结合模块化营养干预对产妇自我效能及母乳喂养率的影响

Effect of IKAP Model Nursing combined with Modular Nutrition Intervention on Maternal Self-efficacy and Breastfeeding Rate

目的探讨IKAP模式护理结合模块化营养干预对产妇自我效能及母乳喂养率的影响。方法从2018年1月至2018年10月足月分娩的初产妇中选取102例为研究对象,随机分为对照组与观察组各51例,对照组采用常规产后护理,观察组在对照组基础上实施IKAP模式护理结合模块化营养干预,对两组产妇产后自我效能、自护能力、乳汁分泌情况、母乳喂养率进行观察。结果两组产妇产后3 d的自我效能评分无明显差异(P>0.05),产后4周时,观察组产妇的自我效能评分均较对照组高(P<0.05);两组产妇产后3 d的自护能力各维度评分无明显差异(P>0.05),观察组产后4周时的自护能力各维度评分明显高于对照组(P<0.05);观察组产妇泌乳始动时间、产后72 h泌乳量、血清泌乳素水平与对照组比较有明显差异(P<0.05);观察组产后1个月、3个月的母乳喂养率明显较对照组高(P<0.05)。结论IKAP模式护理结合模块化营养干预能提高初产妇产后自我效能及自护能力,也能促进乳汁分泌,提高母乳喂养率,值得推广。

Objective To explore the effects of IKAP model nursing combined with modular nutrition intervention on maternal self-efficacy and breastfeeding rate.Methods 102 cases were selected from the primiparas who gave birth at full term from January 2018 to October 2018.They were randomly divided into control group and observation group,51 cases each.The control group used routine postpartum care.The observation group implemented IKAP mode nursing combined with modular nutrition intervention on the basis of the control group.Postpartum self-effi cacy,self-care ability,milk secretion,and breastfeeding rate were observed in the two groups.Results There was no signifi cant difference in self-effi cacy scores between the two groups(P>0.05).At 4 weeks postpartum,the self-effi cacy scores of the women in the observation group was higher than those in the control group(P<0.05).There was no signifi cant difference in the scores of self-protection ability between the two groups(P>0.05).The scores of self-care ability in the observation group at 4 weeks postpartum was significantly higher than those in the control group(P<0.05).The starting time of lactation,the amount of lactation at 72 hours postpartum,and the level of serum prolactin in the observation group were signifi cantly different from those in the control group(P<0.05).The breastfeeding rate of the observation group at 1 month and 3 months after delivery was signifi cantly higher than that of the control group(P<0.05).Conclusion IKAP mode nursing combined with modular nutrition intervention can improve postpartum self-effi cacy and self-care ability of primipara,and also promote milk secretion and increase breastfeeding rate,which is worth promoting.
