新冠肺炎抗疫一线ICU医护人员失眠状况及影响因素调查

Insomnia among ICU healthcare woikers on front-line against COVID-19 and related influence factors

目的了解武汉市新冠肺炎症监护室医护人员的失眠状况及严重程度,并探索影响因素,为制定针对性改善措施提供依据和参考.方法采用自行设计的一般资料问卷、失眠严重指数量表和焦虑自评量表对武汉市、重庆市5所新冠肺炎定点收治医院的125名ICU医护人员进行匿名调查.结果(1)有51例(40.8%)ICU医护工作者表现出不同程度的失眠症状,排名前三位的失眠症状为:对睡眠模式的不满意度、早醒和对失眠的焦虑.因工作年限、总体健康状况、压力感、积极应对能力、疲劳感和焦虑情绪的不同,其失眠严重指数间均有显著性的差异(P<0.05).(2)失眠严重指数与压力感、疲劳感、焦虑情绪、总体健康状况呈现中等强度的显著正相关(0.4<r<0.6,P<0.01);与积极应对能力呈现弱强度的显著负相关(r=-0.22,P<0.01).(3)不良的总体健康状况、疲劳感和焦虑情绪可有效预测失眠严重程度,回归系数R=0.67,R2=0.45,总解释率为45%.结论抗疫一线ICU医护人员失眠状况不理想,需尽早从多方面着手提供干预措施.

Objective To explore the situation and severity of insomnia among frontline intensive care unit(ICU)healthcare workers in designated hospitals against COVID-19 in Wuhan and the influence factors,so as to provide reference and basis for preparing targeted improvement measures.Methods A self-administered general questionnaire,insomnia severity index(ISI)and selfrating anxiety scale(SAS)were used among 125 ICU healthcare workers in five designated hospitals against COVID-19 in Wuhan and Chongqing.Results(1)A total of 51(40.8%)participants showed different degrees of insomnia symptoms.And the top three insomnia symptoms were:dissatisfaction with sleep pattern,early awakening and anxiety over insomnia.Significant differences were found on ISI scores according to different working years,levels of general health condition,stress,ability of positive response,fatigue and anxiety(P<0.05).(2)The ISI scores were moderately and positively correlated with the stress,fatigue,anxiety and general health(0.4<r<0.6,P<0.01)and weakly and negatively correlated with the ability of positive response(r=-0.22,P<0.01).(3)Poor health condition,fatigue and anxiety were predictors for insomnia severity,with regression coefficient R=0.67,R^2=0.45,and an overall interpretation rate of 45%.Conclusion Frontline ICU healthcare workers are suffering from severe insomnia,and early interventions should be provided as early as possible.
