摘要
目的探讨CT动脉期强化特点与肝细胞癌(HCC)病理分化程度、增殖细胞核抗原(PCNA)、磷酸酯酶与张力蛋白同源物(PTEN)的表达及预后之间的关系.方法观察79例HCC患者CT动脉期特点;HE染色检测HCC组织分化程度;免疫组化检测PCNA和PTEN的表达;统计分析CT动脉期特点(肿瘤大小、包膜情况、血管强化特点、肿瘤坏死情况、侵袭转移情况和肿瘤供血类型)与HCC分化程度、PCNA、PTEN的表达及患者预后的相关性.结果肿瘤大小、血管强化特点、侵袭转移情况和肿瘤供血类型与HCC分化程度、PTEN的表达呈负相关,与PCNA的表达呈正相关;包膜完整情况、肿瘤坏死情况与HCC分化程度、PTEN的表达呈正相关,与PCNA的表达呈负相关;肿瘤>3cm和有侵袭转移的患者预后较差.结论HCC的CT动脉期特点可在一定程度上反映肿瘤分化程度、PCNA、PTEN的表达及预后情况.
Objective To explore the correlation between the arterial phase enhanced CT image findings and degree of pathological differentiation of hepatocellular carcinoma(HCC),expression of proliferating cell nuclear antigen(PCNA)and phosphatase and tensin homolog(PTEN)and prognosis.Methods A total of 79 patients with HCC were observed with respect to arterial phase CT image findings.HE staining was used to detect the degree of differentiation of HCC tissues and immunohistochemistry(IHC)was perfonned to detect the expression of PCNA and PTEN.A statistical analysis was made for the correlation between the arterial phase CT image findings(tumor size,envelope,vascular characteristics with enhanced CT scan,tumor necrosis,invasion and metastasis,and type of tumor blood supply)and differentiated degree of HCC,expression of PCNA and PTEN and patient prognosis.Results The tumor size,vascular characteristics with enhanced CT scan,invasion and metastasis,and type of tumor blood supply showed a negative correlation with differentiated degree of HCC and expression of PTEN,and a positive correlation with expression of PCNA.The envelope integrity and tumor necrosis were positively correlated with differentiated degree of HCC and expression of PTEN,and negatively correlated with expression of PCNA.Patients with tumor size of>3 cm and invasion and metastasis had a poor prognosis.Conclusion The arterial phase CT image findings of HCC can reflect the degree of tumor differentiation,expression of PCNA and PTEN and prognosis to some extent.
